乳房自体组织重建的研究进展

Advances in research on autologous breast reconstruction

乳腺癌是全世界女性最常见的恶性肿瘤。这些患者必须面对手术治疗对其生活质量和身体外观的长期影响。因此,越来越多的患者亟需一种结合肿瘤治疗与乳房重建的方法来满足其对疾病的治疗和乳房外观的需求。在美国,20世纪80年代接受乳房切除术后即刻重建的患者只有不到20%。但近年来,越来越多的患者选择即刻重建。乳房自体组织重建因其美容效果好而成为了许多患者的首选方案,但其术前穿支血管的选择及术后并发症的发生仍是目前自体组织重建所面对的问题。本文就乳房重建技术中自体组织重建皮瓣的应用、相关并发症的发生及预防、皮瓣穿支血管的影像学评估及腔镜技术下乳房重建的研究进展进行了详细的总结。

Breast cancer is the most common malignancy in women worldwide.These patients must face the long-term effects of surgical treatment on their quality of life and physical appearance.Therefore,more and more patients are in urgent need of a combination of tumor treatment and breast reconstruction to meet their needs for treatment of the disease and breast appearance.In the United States,less than 20%of patients were reconstructed immediately after mastectomy in the 1980 s.But in recent years,more and more patients have chosen to reconstruct immediately.The reconstruction of breast autologous tissue has become the first choice for many patients because of its good cosmetic effect.However,the choice of preoperative perforating vessels and the occurrence of postoperative complications are still the problems faced by autologous tissue reconstruction.In this paper,the application of autologous tissue reconstruction flap in breast reconstruction technique,the occurrence and prevention of related complications,the imaging evaluation of the perforator of the flap and the breast reconstruction under the endoscopic technique are summarized in detail.
