血清C-反应蛋白联合细胞角蛋白19片段抗原、鳞状上皮细胞癌相关抗原及神经元特异性烯醇化酶检测在食管癌患者中的临床应用

Clinical application of combined detection of serum CRP,CYFRA21-1,SCC and NSE in esophageal cancer

目的探讨C-反应蛋白(C-reactive protein,CRP)联合血清肿瘤标志物细胞角蛋白19片段抗原(CYFRA21-1)、鳞状上皮细胞癌相关抗原(SCC)及神经元特异性烯醇化酶(NSE)在食管癌患者中的表达变化及临床诊断价值。方法选取食管癌患者132例与健康体检者80例,检测其CRP、CYFRA21-1、SCC及NSE表达水平,分析食管癌不同TNM分期组间差异;采用受试者工作特征曲线(ROC)比较各指标诊断价值。结果食管癌组CRP、CYFRA21-1、SCC及NSE的检测水平及阳性率均高于对照组(P <0. 05),并随着临床分期、肿瘤浸润深度的增加而显著增加(P <0. 05)。4项指标联合检测灵敏度为81. 30%,特异度为85. 45%,AUC值为0. 865。结论 CRP联合CYFRA21-1、SCC及NSE可提高检测灵敏度,在食管癌鉴别诊断、临床分期及术后效果评估等方面具有重要的临床应用价值。

Objective To investigate the value of dynamic monitoring of CRP,CYFRA21-1,SCC and NSE in esophageal cancer patients.Methods A total of 132 cases in esophageal cancer group and 80 cases in control group were recruited.The levels of CRP,CYFRA21-1,SCC and NSE were detected and the results were compared to analyze the differences among the groups with different TNM stages of esophageal cancer.Receiver operator characteristic curve(ROC)was used to compare the diagnostic value of each indicator.Results Serum levels and positive rates of CRP,CYFRA21-1,SCC and NSE in esophageal cancer patients were significantly higher than those in healthy subjects.The differences in levels of CRP,CYFRA21-1,SCC and NSE were statistically significant between esophageal cancer patients and healthy control group(P<0.05),and increased gradually with the clinical stage of esophageal cancer.The combined detection sensitivity of the four indexes was 81.30%,the specificity was 85.45%,and the AUC value was 0.865.Conclusion Combined detection of CRP,CYFRA21-1,SCC and NSE could increase the sensitivity for the diagnosis of esophageal cancer.It can be used as the early diagnosis of esophageal important auxiliary.
