XUE Bo-yu’s five methods for prevention and treatment of hepatic encephalopathy

薛博瑜教授智圆行方,兼收并蓄,匠心独运,治验颇丰,薛师认为肝性脑病发病主要在肝,涉及心、脾、肾、大肠、脑多个脏腑,病理因素包括风、火、痰、虚、毒,病机包含肝体不舒,阳郁生风;火热炽盛,内闭心包;痰湿内盛,蒙蔽神窍;肝肾阴亏,虚阳上扰;肠毒内生,神明暓乱5个方面。故临证时常采用疏肝息风、清心开窍、健脾化湿、滋阴潜阳、通腑泄浊五法以防治肝性脑病,并时时重视疏肝养肝,以肝为本,却不囿于一脏一因,整体出发,兼顾脏腑,虚实标本,衷中参西,病证结合,辨证治之。

Professor XUE Bo-yu is wise and gracious,who has unique views and extensive academic experience.Professor XUE believes that the incidence of hepatic encephalopathy is mainly in the liver,involving multiple organs:heart,spleen,kidney,large intestine and brain.Pathological factors include wind,fire,phlegm,deficiency and poison.The pathogenesis of TCM of HE comprises five aspects:liver qi stagnation and yang depression forming wind,heat flourishing and closing pericardium inside,accumulation of phlegm-damp and blocking the spiritual aperture,yin deficiency in the liver and kidney and deficiency yang harassing upper,endogenous intestinal toxicity and disorder of cerebral soul.Therefore,soothing the liver for calming endogenous wind,clearing heart for inducing resuscitation,invigorating spleen for eliminating dampness,nourishing yin for suppressing hyperactive yang,clearing the hollow viscera for eliminating turbidity are five methods to prevent and treat hepatic encephalopathy.Meanwhile,professor XUE always pays attention to relieving liver and nourishing the liver,basing on the liver,but does not limit to one viscera or cause,setting out with the whole,taking zang-fu viscera into account,tackling both the root causes as well as the manifestations,integrating traditional Chinese and Western medicine,combining disease and syndrome,treating with syndrome differentiation.
