张虹“宣通冲门法”治疗下肢顽痹经验

Experience of ZHANG Hong in the treatment of obstinate arthralgia of lower extremities by‘dispelling Chongmen method’

下肢顽痹多病程长,顽固不愈,在临床上治疗难度大。张虹教授在经络、八纲、脏腑辨证指导下,使用“宣通冲门法”贯穿治疗下肢顽痹的始终,达到疏通经络、宣通经气的目的。“宣通冲门法”以“冲门穴”为依托,根据患者下肢顽痹阴面阳面(内外侧)疼痛的偏重,选择临床针刺深度、行针手法、是否留针、是否温针灸等。手法操作过程强调押手辅助作用和手法成功标志“气至”。总结张虹教授临床经验结合广泛文献学习,论证并形成“宣通冲门法”的标准操作,供广大医者学习。解决目前“冲门穴”定位具有争议、操作危险性大、医家使用频率较少的问题;亦可扩大“冲门穴”临床使用范围,为临床治疗下肢顽痹提供新思路。

It is difficult to treat lower extremity obstinacy due to its long course and obstinacy.Under the guidance of syndrome differentiation of meridians,eight elements and zang-fu organs,Professor ZHANG Hong used the‘dispelling Chongmen method’throughout the treatment of obstinate paralysis of lower limbs,so as to promoting menstruation and channel qi.Chongmen(SP12)is the key of dispelling chongmen method,clinical acupuncture depth,needle manipulation,needle retention and warm acupuncture were selected according to the pain in the yin and yang(inner and outer)sides of the patients’lower extremities.The manipulation process emphasizes the auxiliary function of the hand and the successful manipulation marks‘arrival of qi’.This paper summarized Professor ZHANG Hong’s clinical experience and combined with extensive literature study,demonstrated and formed the standard operation of‘dispelling Chongmen method’,which can be learned by doctors.To solve the controversial ideas about Chongmen(SP12)that the operation is of large risk and less frequency.It can also expand the clinical application scope of Chongmen(SP12),and provide a new idea for the clinical treatment of lower limb obstinate arthralgia.
