摘要
目的分析周围型肺癌不同感兴趣区(ROI)及转移性纵隔淋巴结的能谱参数,探索周围型肺癌与转移性纵隔淋巴结同源性匹配最佳的ROI位置。资料与方法回顾性分析40例行能谱CT增强扫描的周围型肺癌合并纵隔淋巴结转移的影像学资料。对周围型肺癌的中央、周边、整体及纵隔淋巴结的中央分别勾画ROI,测量周围型肺癌不同位置ROI及纵隔淋巴结内的水(碘)含量、碘(水)含量、能谱曲线及其斜率,并分析其相关性。结果周围型肺癌不同ROI内的碘(水)含量及能谱曲线斜率:周边>整体>中央,差异均有统计学意义(P<0.05)。周围型肺癌不同ROI的碘(水)含量与转移性纵隔淋巴结的碘(水)含量均呈高度相关,相关程度依次为:周边(r=0.95,P<0.05),整体(r=0.86,P<0.05),中央(r=0.83,P<0.05)。周围型肺癌不同ROI的能谱曲线斜率与转移性纵隔淋巴结的能谱曲线斜率均呈高度相关,相关程度依次为:周边(r=0.95,P<0.05),整体(r=0.94,P<0.05),中央(r=0.83,P<0.05)。结论周围型肺癌不同ROI内的碘(水)含量及能谱曲线斜率具有差异,周围型肺癌周边的碘(水)含量和能谱曲线斜率与转移性纵隔淋巴结的碘(水)含量和能谱曲线斜率相关性最高。
Purpose To explore the best regions of interest(ROI)location for homology matching between peripheral lung cancer and metastatic mediastinal lymph nodes by analyzing the energy spectrum parameters of different ROI of peripheral lung cancer and metastatic mediastinal lymph nodes.Materials and Methods The imaging data of 40 patients with peripheral lung cancer complicated with metastatic mediastinal lymph nodes who underwent energy spectrum CT enhanced scan were retrospectively analyzed.The ROIs of the central,peripheral,whole of peripheral lung cancer and mediastinal lymph nodes were delineated respectively,the water(iodine)content,iodine(water)content,energy spectrum curve and its slope were measured and analyzed correlatedly.Results Iodine(water)content and the slope of energy spectrum curve in different ROIs of peripheral lung cancer:periphery>whole>center,with statistically significant difference(P<0.05).Iodine(water)content in different ROIs of peripheral lung cancer was highly correlated with iodine(water)content in metastatic mediastinal lymph nodes,with the correlation degree in the order of periphery(r=0.95,P<0.05),whole(r=0.86,P<0.05),center(r=0.83,P<0.05).The slope of the spectral curve of different ROIs of peripheral lung cancer was significantly correlated with the slope of the spectral curve of metastatic mediastinal lymph nodes,with the correlation degree of in the order of periphery(r=0.95,P<0.05),whole(r=0.94,P<0.05),center(r=0.83,P<0.05).Conclusion Iodine(water)content and the slope of energy spectrum curve in different regions of peripheral lung cancer are different.Iodine(water)content and the slope of energy spectrum curve around peripheral lung cancer have the highest correlation with iodine(water)content and the slope of energy spectrum curve of metastatic mediastinal lymph nodes.
