慢性阻塞性肺疾病患者吸入给药治疗自我体验的质性探究

Qualitative Study on Self-experience of Patients with Chronic Obstructive Pulmonary Disease Treated by Inhalation

了解慢性阻塞性肺疾病(COPD)患者行吸入给药治疗时的情感自我体验。采用质性研究中现象学的研究方法,通过半结构访谈法,对18名COPD患者进行深入访谈,将访谈资料行逐字转录后根据Colaizzi现象学分析法进行分析。根据分析结果,对COPD患者在吸入给药治疗时提炼出以下四个主题:1)吸入给药治疗引起的舒适感下降;2)慢性的妥协;3)对吸入给药效果的不确定感;4)自我负性情绪增加。结果表明,COPD患者在治疗过程中具有明显的负性体验,因吸入给药引起的不适、认知不足及应对方式缺乏有效性及疾病周期长的缘故,导致其在心理上的妥协,无法长期坚持,从而使患者对吸入给药治疗的依从性变差,需要特别关注该类患者对吸入给药的感受,及时采取干预措施。

The Self-experience emotional experience of patients with COPD undergoing inhalation therapy was investigated in this paper.The phenomenological research methods were used in qualitative research.Through semi-structured interviews,18COPD patients were interviewed in depth.At the end of the interview,the data lines were transcribed verbatim and analyzed according to the Colaizzi phenomenological analysis method.For COPD patients,the following four themes were extracted during inhalation treatment:1)Decrease in the comfort from inhalation therapy;2)Chronic compromise;3)Uncertainty about the effect of inhalation;4)Increasing self-negative emotions.As was shown in the results,patients with COPD had obviously negative experiences in the treatment.In addition,due to the discomfort of inhalation,lack of cognition and effective countermeasures,as well as long disease cycle,patients compromise psychologically,and are unable to undertake long-term adherence to it,which caused that the compliance of patients declined.Special attention should be paid to the feelings of such patients about inhalation.The intervention measures should be taken immediately.
