重庆地区2020年新型冠状病毒肺炎患者209例中医证型调查分析

Study of TCM Syndrome in 209 Novel Coronavirus Pneumonia Cases of Chongqing in 2020

目的回顾性研究分析重庆市新型冠状病毒肺炎(新冠肺炎)患者临床资料,探讨新冠肺炎中医证型特点和病机,为进一步制定科学、高效的救治方案提供依据。方法建立来自重庆市4个定点集中救治医疗机构收治的209例新冠肺炎患者的诊疗信息数据库,分析新冠肺炎的流行病学特点及中医证型规律。结果209例患者中,发病年龄段以中青年为主,大多以咳嗽、发热为临床症状首发表现,分别占70.81%、55.50%;其次是乏力、腹泻、纳差、口干等消化道症状。舌象分析:舌质红占47.33%,舌苔白腻或黄腻占64.67%。证型分布:邪热壅肺证比例最高,占30.14%,其次是湿邪中阻证、寒湿郁肺证,分别占16.27%、11.96%,此3个证型总计占比高达58.37%。结论重庆地区新冠肺炎患者可能以输入性为主。临床证型以邪热壅肺证、湿邪中阻证、寒湿郁肺证为主,其基本病机可能与"湿"和"热"密切相关,随着疾病的进展还会出现"毒""瘀"和"虚"等特点。

Objective:To analyze the characteristics of TCM syndrome of novel coronavirus pneumonia(NCP) in Chongqing by retrospective analysis,providing the reference for further formulating a scientific and efficient treatment plan.Methods:The characteristics of TCM syndrome and the epidemiological characteristics of NCP were analyzed by establishing the clinical information database of 209 cases in 4 designated medical institutions in Chongqing.Results:Among the 209 patients,the main age group was young and middle-aged,with cough and fever as the first symptoms,accounting for 70.81%and 55.50%,respectively,followed by fatigue,diarrhea,anorexia mouth,dry and other gastrointestinal symptoms.Tongue analysis showed that red tongue accounted for 47.33%,white or yellow greasy tongue coating for 64.67%.In the distribution of TCM syndromes,the proportion of the syndromes of pathogenic heat obstructing lung was the highest as 30.14%,followed by the syndromes of dampness obstructing and the syndromes of cold dampness stagnating lung accounting for 16.27%and 11.96%respectively,and the total of these three syndromes was 58.37%.Conclusion:There could be mainly imported NCP cases in Chongqing.The clinical TCM syndrome of NCP patients in Chongqing are mainly pathogenic heat obstructing lung syndrome,dampness obstructing syndrome and cold dampness stagnating lung syndrome.The basic pathogenesis may be closely related to dampness and heat.Toxicity,stasis and deficiency will appear with the progress of the disease.
