临床药师在肾内科病区开展医嘱审核工作

Practice of drug use intervention by clinical pharmacists in nephrology department

目的分析某三甲医院肾内科病区医嘱审核的情况,为临床药师参与肾内科医嘱审核提供参考。方法2017年9月至2018年8月成都医学院第一附属医院临床药师结合每日查房对肾内科病区医嘱实行审核,并对审核情况进行总结分析。结果临床药师共审核肾内科医嘱10383条,其中不合理医嘱156条,不合理率1.50%。不合理医嘱类别主要为药物的用法用量与疗程(29.49%)、重复用药(25.00%)、药物选择(14.74%)。临床药师审核干预后,不合理率从2.26%下降至0.89%。结论临床药师通过对病区医嘱的审核干预,促进临床合理用药,从而保证临床用药的安全性和有效性。

Objective To analyze the pharmaceutical intervention in nephrology department of a three-level hospital,and to provide reference for drug use intervention by clinical pharmacists in nephrology department.Methods From September 2017 to August 2018,clinical pharmacists of The First Affiliated Hospital of Chengdu Medical College reviewed doctors’prescriptions after daily ward rounds in nephrology department and analyzed the status.Results A total 10383 pieces of medical prescriptions were reviewed,among which 156 were irrational,accounting for 1.5% of all examined prescriptions.The types of irrational medical prescriptions included improper dosage and course of treatment(29.49%),drug duplication(25.00%),and improper choice of drug(14.74%).After the clinical pharmacists reviewed the intervention,the rate of irrational drug use dropped from 2.26% to 0.89%.Conclusion Clinical pharmacists should provide effective intervention,promote rational use of drugs,and thus improve the efficacy and safety of clinical drug use.
