中西医结合治疗功能性下丘脑性闭经临床观察

Clinical Observation on Integrated Chinese-Western Therapy in Treatment of Functional Hypothalamic Amenorrhea

目的观察中西医结合治疗功能性下丘脑性闭经(FHA)的临床疗效。方法130例FHA患者以随机数字表法分为对照组(65例)和观察组(65例),对照组进行雌孕激素序贯疗法、营养支持、调节生活方式、心理疏导治疗,观察组在对照组治疗的基础上,同时联合中药针灸治疗,共治疗3个月,疗程结束后随访6个月。比较两组的治愈率、有效率、复发率及两组治疗前后身体质量指数(BMI),血清促卵泡激素(FSH)、促黄体生成素(LH)、雌二醇(E2)水平。结果观察组治愈率53.80%、有效率86.20%,对照组治愈率35.40%、有效率70.77%,观察组治愈率、有效率显著高于对照组,差异均有统计学意义(P<0.05);观察组复发率13.85%明显低于对照组复发率15.38%,但无统计学意义(P>0.05);治疗后两组BMI、FSH、LH、E2均较治疗前明显升高,观察组明显高于对照组,差异均有统计学意义(P<0.05)。结论中西医结合方法综合治疗FHA可显著增加患者BMI,升高血清FSH、LH、E2水平,提高疗效。

Objective To observe the clinical effect of integrated Chinese-western therapy on functional hypothalamic amenorrhea(FHA).Methods 130 cases of patients with FHA were randomly divided into control group and observation group,65 cases each.Control group received hormone sequential therapy,nutritional support,lifestyle adjustment and psychological counseling,observation group was treated with traditional Chinese medicine combined with acupuncture and moxibustion on the basis of control group,both groups were treated for 3 months and followed up for 6 months.The cure rate,effective rate,recurrence rate,body mass index(BMI),serum levels of follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)before and after treatment were compared between the two groups.Results The cure rate and effective rate of observation group and control group were 53.80%and 86.20%,35.40%and 70.80%,respectively,which of observation group were significantly higher than those of control group with statistical significance(P<0.05);the recurrence rate of observation group was 25.70%,lower than that of control group(43.50%)without statistical significance(P>0.05);after treatment,the levels of BMI,FSH,LH,E2 in observation group were significantly higher than those in control group,both groups were significantly enhanced than those before treatment,and the difference was statistically significant(P<0.05).Conclusion Comprehensive treatment of FHA with integrated Chinese-western therapy can significantly increase BMI,serum levels of FSH,LH,E2,and improve the curative effect of FHA.
