重度子痫前期患者脐动脉血流变化情况及其与不良妊娠结局的相关性分析

The Analysis of Changes of Umbilical Artery Blood Flow in Patients with Severe Preeclampsia and Its Correlation with Adverse Pregnancy Outcome

目的探讨重度子痫前期患者脐动脉血流变化情况及其与不良妊娠结局的关系。方法选取2016年1月—2018年12月辽油宝石花医院收治的67例重度子痫前期患者作为观察组,选择同期无妊娠期高血压疾病的149例妊娠女性作为对照组,比较两组脐血流相关指标及不良妊娠结局发生情况。结果观察组脐血流相关指标收缩期峰值流速/舒张末期流速≥3.0、搏动指数≥1.5及阻力指数≥0.8者所占比例高于对照组,新生儿窒息、胎儿生长受限、胎儿窘迫、低体重儿发生率高于对照组,差异均有统计学意义(P<0.05)。相关分析显示,脐血流指标与不良妊娠结局的发生呈正相关(P<0.05)。结论重度子痫前期患者脐动脉血流出现异常变化,并且这种变化与不良妊娠结局呈正相关。

Objective To investigate the changes of umbilical artery blood flow in patients with severe preeclampsia and its correlation with adverse pregnancy outcome.Methods A total of 67 patients with severe preeclampsia admitted in Liao Oil Gem Flower Hospital from January 2016 to December 2018 were collected as the observation group.The 149 pregnant women who did not have pregnancy induced hypertension in the same period were selected as the control group.The indexes of cord blood flow and pregnancy outcomes between two groups were compared.Results The proportion of peak systolic velocity/end diastolic velocity≥3,pulsatility index≥1.5 and resistance index≥0.8 in the observation group was significantly higher than those in the control group,the incidence of neonatal asphyxia,fetal growth restriction,fetal distress and low weight infants was significantly higher than those of the control group(P<0.05).The correlation analysis showed that umbilical blood flow index was positively correlated with adverse pregnancy outcome(P<0.05).Conclusion The abnormal changes of umbilical artery blood flow in patients with severe preeclampsia are related to the adverse pregnancy outcome.
