后路短节段减压融合内固定术治疗腰椎退行性侧弯并椎管狭窄症的短期效果分析

Analysis of short-term effect of posterior short-segment decompression and fusion in treatment of lumbar degenerative scoliosis combined with spinal stenosis

目的探讨后路短节段减压融合内固定术治疗腰椎退行性侧弯(DLS)并椎管狭窄症的效果。方法选取2017年1月—2018年12月手术治疗并获得随访的34例DLS合并椎管狭窄症患者,全部患者均接受后路短节段减压融合内固定术,统计术中出血量、手术用时及并发症情况,对比手术前后患者Oswestry功能障碍指数(ODI)评分、腰部及腿部疼痛视觉模拟(VAS)评分、腰椎Cobb角、腰椎前凸角评估手术治疗效果。结果34例患者手术均顺利完成,和术前相比,患者症状得到明显缓解,腰部VAS评分、腿部部VAS评分以及0DI评分均得到明显改善和维持,腰椎Cobb角、腰椎前凸角均有所改善。结论对于腰椎退行性侧弯并椎管狭窄症且腰椎Cobb角<30°的老年患者,后路短节段减压融合内固定术具有良好的临床效果。

Objective To explore the effect of posterior short-segment decompression and fusion on lumbar degenerative scoliosis(DLS)complicated with spinal stenosis.Methods Totally 34 patients with lumbar degenerative scoliosis and spinal stenosis who underwent surgical treatment from January 2017 to December 2018 were enrolled.The time of surgery,intraoperative blood loss and complications were compared.The visual analogue scale(VAS),Oswestry disability index(ODI),lumbar Cobb angle,lumbar lordosis were compared.Results Surgery for all patients completed successfully.Compared with preoperative,the patients’VAS and 0 DI were significantly reduced and maintained,and the lumbar Cobb angle and lumbar lordosis were ameliorated.Conclusion For elderly patients with lumbar degenerative scoliosis complicated with spinal stenosis and lumbar Cobb Angle<30°,posterior short-segment decompression and fusion internal fixation can achieve good clinical results and improve the quality of life of patients.
