基于"互联网+"的高血压管理服务模式对高血压患者血压管理效果观察

The effect of"Internet+"-based hypertension management on blood pressure in patients with hypertension

目的探讨基于"互联网+"的高血压管理服务模式对高血压患者的健康管理效果.方法选取2016年9月至2017年2月重庆医科大学附属第一医院高血压患者110例分为平台管理组和对照组.平台管理组接受健康管理平台管理,管理流程包括建立电子健康档案并评估、制定健康计划、患者自测血压等指标并上传平台、阶段性评估、健康宣教等;对照组接受门诊常规管理,随访12周.比较两组干预前后血压、血压控制率、服药依从性、生活质量、体力活动改变.结果随访干预12周后,两组的血压水平均较入组时下降,平台管理组收缩压、舒张压分别为(132.7±9.9)mm Hg、(80.6±9.9)mm Hg,对照组分别为(140.7±14.2)mm Hg、(77.6±14.3)mm Hg,但平台管理组收缩压下降更显著(P<0.05).平台管理组血压控制率(64.3%)明显高于对照组(33.3%),差异有统计学意义(P<0.05).单因素和多元线性回归分析均显示,平台管理与收缩压变化值显著相关,回归系数为-8.9.两组服药依从性评价、12条目简易健康量表(SF-12)评分、体力活动水平差异无统计学意义(P>0.05).结论基于"互联网+"的高血压管理服务模式可以显著降低高血压患者收缩压水平,提高血压控制率.

Objective To explore the effect of a hypertension-management model based on"Internet+"on the health management of hypertensive patients.Methods A total of 110 hypertensive patients were recruited from the First Affiliated Hospital of Chongqing Medical University between September 2016 and February 2017 and assigned to the study group or control group.The study group received management from a health management platform and the process included the establishment and evaluation of electronic health files,the formulation of health plans,the self-test of blood pressure by patients and the uploading of the results to the platform,periodic assessment,health education,etc.The control group underwent regular hypertension education and outpatient follow-up.A comparison of blood pressure level,rate of blood pressure control,drug compliance,quality of life and physical activity was carried out 12 weeks later.Results After 12 weeks of follow-up,the systolic blood pressure and diastolic blood pressure were(132.7±9.9)mm Hg and(80.6±9.9)mm Hg in study group and(140.7±14.2)mm Hg and(77.6±14.3)mm Hg in the control group;the decrease in systolic blood pressure in the study group was more obvious than that in the control group(P<0.05).The rate of blood pressure control in the study group(64.3%)was significantly higher than that in the control group(33.3%)and the difference was significant(P<0.05).Both the single factor and multiple linear regression analysis showed that the platform management was significantly correlated with systolic blood pressure and the regression coefficient was-8.9.There was no significant difference in drug compliance,quality of life and physical activity between the two groups(P>0.05).Conclusion The service model of Internet-based hypertension management can significantly reduce systolic blood pressure level and improve the rate of blood pressure control among hypertensive patients.
