急性与慢性阑尾炎超声表现以及病理分型的比较分析

Comparative Analysis of Ultrasound Manifestations and Pathological Typing of Acute and Chronic Appendicitis

目的分析急性与慢性阑尾炎超声表现以及病理分型的比较。方法方便选取该院于2017年1月—2018年12月收治的150例疑似阑尾炎患者的临床资料,对比急性、慢性阑尾炎超声表现与病理分型。结果在所选150例阑尾炎患者之中,150例患者经病理诊断确诊为阑尾炎患者,超声诊断出135例为阑尾炎患者,诊断符合率为90.00%。诊断病变阑尾位置如下,90例患者病变位置在回肠前位,占比60.00%,60例患者病变位置为回肠后位,占比40.00%;病理诊断在150例阑尾炎患者中,病理诊断急性单纯性的阑尾炎患者、急性化脓性阑尾炎、急性坏疽性阑尾炎并阑尾周围脓肿分别是30例、80例、10例,超声诊断急性单纯性的阑尾炎患者22例(73.33%),急性化脓性阑尾炎75例(93.75%),急性坏疽性阑尾炎并阑尾周围脓肿10例(100.00%);病理诊断慢性阑尾炎患者一共30例,超声诊断21例为慢性阑尾炎,占比70.00%。结论在阑尾炎诊断、病例分型中应用超声诊断符合率比较高。

Objective To analyze the comparison of ultrasound manifestations and pathological typing between acute and chronic appendicitis.Methods Clinical information of 150 patients with suspected appendicitis in the hospital from January 2017 to December 2018 were convenient selected and analyzed.Ultrasound manifestations and pathological types of acute and chronic appendicitis were compared.Results Among 150 selected patients with appendicitis,150 patients were diagnosed with appendicitis by pathological diagnosis,and 135 patients with appendicitis were diagnosed by ultrasound.The diagnostic coincidence rate was 90.00%.The location of the appendix diagnosed was as follows.The location of the lesion was 90 cases in the anterior ileum,accounting for 60.00%,and the location of the lesion in the 60 cases was posterior,accounting for 40.00%.The pathological diagnosis was acute simple in 150 patients with appendicitis.The number of patients with appendicitis,acute suppurative appendicitis,acute gangrenous appendicitis and periappendic abscesses were 30 cases,80 cases,and 10 cases,respectively.Ultrasound diagnosed 22 cases(73.33%)of acute simple appendicitis and 75 cases of acute suppurative appendicitis(93.75%),10 cases(100.00%)with acute gangrenous appendicitis and periappendic abscess;a total of 30 patients with pathological diagnosis of chronic appendicitis and 21 cases of chronic appendicitis with ultrasound diagnosis,accounting for 70.00%.Conclusion The ultrasonic diagnostic diagnosis rate is high in the diagnosis and case classification of appendicitis.
