微小切口进路坚固内固定术治疗口腔颌面部骨折的疗效研究

Efficacy of Sturdy Internal Fixation with Micro-incision Approach for Oral and Maxillofacial Fractures

目的对微小切口进路坚固内固定术治疗口腔颌面部骨折的临床效果进行分析和研究。方法方便选择该院在2017年1月—2018年12月期间收治的80例实施手术内固定的口腔颌面部骨折患者作为该次研究纳入的对象,按照患者病历单双号将其划分为两组,对照组40例,观察组40例。通过传统颌间结扎内固定或钛钉牵引术对对照组予以治疗,通过微小切口进路坚固内固定术对观察组予以治疗,对两组患者的治疗效果进行分析和比较。结果对照组中有27例固定未移动,占67.5%,30例咬合功能完全恢复,占75.0%,29例外貌恢复满意,占72.5%,30例I期愈合,占75.0%;观察组中有37例固定未移动,占92.5%,39例咬合功能完全恢复,占97.5%,37例外貌恢复满意,占92.5%,38例I期愈合,占95.0%。观察组的固定未移动率、咬合功能完全恢复率、外貌恢复满意度和I期愈合率均明显优于对照组,两组患者相比差异有统计学意义(χ^2=6.728、6.419、7.581、8.022,P<0.05)。对照组并发症发生率为27.5%,观察组并发症发生率为10.0%,观察组的并发症发生率明显低于对照组,两组患者相比差异有统计学意义(χ^2=7.499,P<0.05)。结论在口腔颌面部骨折的临床治疗中应用微小切口进路坚固内固定术具有显著的临床效果,能够实现良好的外形恢复,而且并发症少。

Objective To analyze and study the clinical effect of micro-incision approach and solid internal fixation for oral and maxillofacial fractures.Methods 80 patients with oral and maxillofacial fractures who underwent surgical fixation between January 2017 and December 2018 were convenient selected as the subjects included in this study.They were divided into two groups according to the single and double numbers of patient records.There were 40 cases in the control group and 40 cases in the observation group.The control group was treated by traditional intermaxillary ligation and internal fixation or titanium nail traction,and the observation group was treated by microincision approach and solid internal fixation.The treatment effects of the two groups were analyzed and compared.Results In the control group,there were 27 cases of immobilization,accounting for 67.5%,30 cases fully recovered occlusal function,accounting for 75.0%,29 cases recovered satisfactorily,accounting for 72.5%,30 cases of stage I healing,accounting for 75.0%;In the observation group,there were 37 cases of immobilization,accounting for 92.5%,39 cases had complete recovery of occlusal function,accounting for 97.5%,37 cases recovered satisfactorily,accounting for 92.5%,and 38 cases had stage I healing,accounting for 95.0%.The observation group's fixed immobile rate,complete recovery rate of occlusal function,satisfaction with appearance recovery,and stage I healing rate were significantly better than those in the control group,and the differences between the two groups were statistically significant(χ^2=6.728,6.419,7.581,8.022,P<0.05).The complication rate in the control group was 27.5%,and the complication rate in the observation group was 10.0%.The complication rate in the observation group was significantly lower than that in the control group.The difference between the two groups was statistically significant(χ^2=7.499,P<0.05).has statistical significane.Conclusion The application of micro-incision and solid internal fixation in the clinical treatment of oral and maxillofacial fractures has significant clinical effects,can achieve good shape recovery,and has few complications.
