摘要
目的探讨分析小儿肺炎支原体肺炎与心肌损害的关系。方法回顾性分析2017年5月—2019年5月时间内,选择142例该院儿科收治的小儿肺炎患者,根据是否为支原体肺炎进行分组,71例符合支原体肺炎的小儿患者为研究组,71例非支原体肺炎的小儿患者为对照组,对两组患者均进行心电图、心肌酶谱的检测,分析两组心电图、临床指标和心肌酶谱的变化,持续顽固心律失常和心衰的发生率。结果研究组LDH(328.67±97.82)U/L、AST(56.58±18.12)U/L、CKMB(32.69±9.17)U/L、CK(505.23±121.25)U/L以及α-HBDH(318.95±100.03)U/L水平比对照组高(t=11.309,13.736,10.186,22.776,13.861,P<0.05),心悸、乏力、早搏、憋气等临床症状与对照组差异有统计学意义(P<0.05)。心电图变化、病理性杂音与对照组差异有统计学意义(P<0.05),持续顽固心律失常和心衰的发生率比对照组高(P<0.05)。结论小儿肺炎支原体肺炎与心肌损害具有直接关系,心肌损害是小儿肺炎支原体肺炎的并发症之一,通过心电图、临床指标以及心肌酶谱的变化,可以对小儿肺炎支原体肺炎是否存在心肌损害进行判断,做到提前预防,并进行针对性的治疗,促进患者康复,因此研究具有积极的临床意义。
Objective To explore the relationship between mycoplasma pneumoniae pneumonia and myocardial damage in children.Methods A retrospective analysis was performed from May 2017 to May 2019.142 pediatric pneumonia patients admitted to the hospital's pediatric department were selected and grouped according to whether they were mycoplasma pneumonia.71 children who were eligible for Mycoplasma pneumonia were the study group.Two children with non-mycoplasma pneumonia were used as the control group.The electrocardiogram and myocardial enzymes were detected in the two groups.The changes of the electrocardiogram,clinical indicators,and myocardial enzymes,the incidence of persistent stubborn arrhythmia and heart failure were analyzed in the two groups.Results LDH(328.67±97.82)U/L,AST(56.58±18.12)U/L,CKMB(32.69±9.17)U/L,CK(505.23±121.25)U/L,andα-HBDH(318.95±100.03)U/L,were higher than those in the control group(t=11.309,13.736,10.186,22.776,13.861,P<0.05).Clinical symptoms such as palpitations,fatigue,premature beats,and belching were statistically significant different from those in the control group(P<0.05).Electrocardiogram changes,pathological murmurs were statistically significant different from those of the control group(P<0.05),and the incidence of persistent stubborn arrhythmia and heart failure was higher than that of the control group(P<0.05).Conclusion Pediatric mycoplasma pneumoniae pneumonia with myocardial damage has a direct relationship,myocardial damage is one of the complications of pediatric mycoplasma pneumoniae pneumonia,by electrocardiogram(ecg)and clinical indicators,as well as the change of myocardial enzyme spectrum,pediatric mycoplasma pneumoniae pneumonia can be to judge the existence of myocardial damage,early prevention,and targeted treatment,promote patients recover,so the research has positive clinical significance.
