疝环填充式与平片式无张力疝修补术在腹股沟疝中的应用

Application of Tension-free Hernioplasty with Hernia Ring Filling and Plain Film in Inguinal Hernia

目的对比疝环填充式与平片式无张力疝修补术在腹股沟疝中的应用效果。方法方便选择2018年1-12月该院收治的腹股沟疝患者作为研究对象72例,以随机数表法将其分为两组,其中对照组与研究组各纳入患者36例。研究组接受疝环填充式无张力疝修补术治疗,对照组接受平片式无张力疝修补术治疗。比较两组术中失血量、手术时间、住院时间,以及术后并发症情况、术后6个月复发情况。结果研究组术中失血量(178.5±20.0)mL、手术时间(57.5±4.6)min、住院时间(5.3±1.2)d与对照组(180.2±22.2)mL、(58.6±4.5)min、(5.5±1.6)d对比差异无统计学意义(t=0.565、0.485、0.865,P>0.05)。研究组术后并发症发生率5.56%低于对照组22.22%,结果对比差异有统计学意义(χ~2=4.181,P<0.05)。随访6个月,两组各有1例(2.78%)复发病例,复发率对比差异无统计学意义(χ~2=0.514,P>0.05)。结论疝环填充式与平片式无张力疝修补术在腹股沟疝患者中的临床疗效相当,但疝环填充式能够减少术后并发症情况,值得临床应用。

Objective To compare the effect of tension-free hernioplasty with mesh filling hernioplasty in the treatment of inguinal hernia. Methods 72 patients with inguinal hernia admitted to the hospital from January 2018 to December 2018 were convenient selected as the study subjects and divided into two groups by random number table method, of which 36 patients were included in the control group and the research group. The study group received tension-free hernia repair with ring filling, while the control group received tension-free hernia repair with plain film. The intraoperative blood loss,operation time, hospitalization time, complications and recurrence of 6 months after operation were compared between the two groups. Results There was no significant difference in blood loss(178.5 ±20.0)mL, operation time(57.5 ±4.6)min,hospital stay(5.3±1.2) d between the study group and the control group(180.2±22.2)mL,(58.6±4.5) min,(5.5±1.6)d(t=0.565, 0.485, 0.865, P>0.05). The incidence of postoperative complications in the study group was 5.56% lower than that in the control group(22.22%). The difference was statistically significant(χ~2=4.181, P<0.05). After 6 months of follow-up, there was 1 case(2.78%) recurrence in each group. There was no significant difference in recurrence rate between the two groups(χ~2=0.514, P>0.05). Conclusion The clinical effect of hernia ring filling is similar to that of tension-free hernioplasty, but hernia ring filling can reduce the complications after operation, which is worthy of clinical application.
