伤椎置钉单节段内固定植骨治疗胸腰段脊柱骨折的临床效果观察 被引量:3

Clinical Effect of Single-segment Internal Fixation with Bone Graft on Injured Spine for Thoracolumbar Fracture

摘要 目的以胸腰段脊柱骨折患者为例,分析评价伤椎置钉单节段内固定植骨治疗的临床效果。方法该次将该院2017年2月—2018年1月收治的76例胸腰段脊柱骨折患者作为研究对象,分成两组,对照组给予通用型脊柱内固定系统治疗,观察组给予伤椎置钉单节段内固定植骨治疗,对比两组临床治疗效果。结果①观察组,手术时间为(121.88±18.60)min、术中出血量为(86.50±9.00)mL;对照组分别为(145.98±17.65)min、(172.40±9.12)mL;观察组手术时间、出血量明显少于对照组。②观察组,术前椎体前缘高度为(52.83±6.32)mm、术后为(92.88±2.54)mm,术前椎体后缘高度为(75.10±3.93)mm、术后为(96.93±1.76)mm,术前Cobb角为(18.49±3.11)°、术后为(3.69±1.25)°;对照组,术前椎体前缘高度为(52.84±6.30)mm、术后为(92.85±2.51)mm,术前锥椎体后缘高度为(75.11±3.90)mm、术后为(96.88±1.72)mm,术前Cobb角为(18.48±3.12)°、术后为(3.70±1.21)°;术前两组在椎体前缘高度、锥体后缘高度、Cobb角方面均差异无统计学意义(t=1.293、1.187、1.235,P>0.05);术后,两组均明显改善,但对比差异无统计学意义(t=1.356、1.297、1.265,P>0.05)。③术后并发症发生率,观察组为0.00%,对照组为10.53%,两组数据差异有统计学意义(χ2=10.298,P<0.05)。结论针对胸腰段脊柱骨折患者,给予伤椎置钉单节段内固定植骨治疗具备明显的疗效。 Objective To analyze and evaluate the clinical effects of single-segment internal fixation and bone grafting for patients with thoracolumbar spine fractures.Methods The 76 patients with thoracolumbar spine fractures who were treated in the hospital from February 2017 to January 2018 were divided into two groups.The control group was treated with a universal spinal fixation system,and the observation group was treated with single-segment internal fixation of bone grafting,to compare the clinical effects of the two groups.Results 1.In the observation group,the operation time was(121.88±18.60)min and the intraoperative blood loss was(86.50±9.00)mL;the control group was(145.98±17.65)min and(172.40±9.12)mL;the observation group had surgery time and bleeding volume were significantly less than those in the control group.2.In the observation group,the anterior vertebral height was(52.83±6.32)mm before operation and(92.88±2.54)mm postoperatively,and the posterior vertebral height was(75.10±3.93)mm before and(96.93±1.76)mm,Cobb angle before operation was(18.49±3.11)°,and postoperative was(3.69±1.25)°;in the control group,the height of the anterior edge of the vertebral body was(52.84±6.30)mm before operation and(92.85±2.51)mm after operation,the height of the posterior edge of the cone vertebra before operation was(75.11±3.90)mm,(96.88±1.72)mm after operation,the Cobb angle before operation was(18.48±3.12)°and(3.70±1.21)°after operation;There were no statistically significant differences in the anterior vertebral height,posterior marginal height,and Cobb angle of the two groups before surgery(t=1.293,1.187,1.235,P>0.05);both groups im proved significantly after surgery,but there was no statistically significant difference in comparison(t=1.356,1.297,1.265,P>0.05).3.The incidence of postoperative complications was 0.00%in the observation group and 10.53%in the control group.There was a statistically significant difference between the two groups(χ2=10.298,P<0.05).Conclusion For patients with thoracolumbar spine fractures,single-segment internal fixation and bone grafting with injured vertebral nails has obvious curative effect.
