大型C型臂X光机下植入永久性人工心脏起搏器的临床疗效及对患者心房颤动和无症状性心房颤动的影响

摘要 目的探究大型C型臂X光机下植入永久性人工心脏起搏器的临床疗效及对患者心房颤动和无症状性心房颤动的影响。方法选取该院2017年1月—2018年12月收治的52行大型C型臂X光机下植入永久性人工心脏起搏器治疗的患者作为研究对象,分析其临床资料,比较治疗前后左心室舒张末期容积指数(EDVI)、左心室收缩末期容积指数(ESVI)、左心室射血分数(LVEF)等心功能指标和SF-36生活质量评分,以及心房颤动和无症状心房颤动所受影响。结果治疗前,EDVI、ESVI、LVEF水平分别为(41.52±5.68)mL/m2、(20.56±2.73)mL/m2、(53.04±8.10)mL/m2;治疗后,EDVI、ESVI、LVEF水平分别为(50.36±7.49)mL/m2、(29.78±3.48)mL/m2、(62.15±9.74)mL/m2,治疗后均高于治疗前,差异有统计学意义(t=6.781、15.032、5.186,P<0.05)。治疗前,心房颤动率、无症状心房颤动率分别为3.85%、3.85%;治疗后,心房颤动率、无症状心房颤动率分别为7.70%、5.77%,治疗后均高于治疗前,但差异无统计学意义(χ2=0.078、0.210,P>0.05)。治疗前,生理职能、躯体疼痛、情感职能、社会功能等评分分别为(51.26±7.79)分、(49.20±7.02)分、(54.61±8.02)分、(52.68±7.84)分;治疗后,生理职能、躯体疼痛、情感职能、社会功能等评分分别为(60.37±8.92)分、(58.74±8.19)分、(62.45±9.36)分、(61.52±9.08)分,治疗后均高于治疗前,差异有统计学意义(t=5.547、6.378、4.587、5.324,P<0.05)。结论大型C型臂X光机下植入永久性人工心脏起搏器,有利于心脏功能的改善和提高患者生活质量,心房颤动和无症状心房颤动发生情况少。 Objective To investigate the clinical effect of implanting a permanent artificial cardiac pacemaker under a large C-arm X-ray machine and its effect on atrial fibrillation and asymptomatic atrial fibrillation.Methods A total of 52 patients underwent permanent artificial cardiac pacemakers underwent 52 large C-arm X-ray machines from January 2017 to December 2018 in the hospital.The clinical data were analyzed and compared before and after treatment.Left ventricular end-diastolic volume index(EDVI),left ventricular end-systolic volume index(ESVI),left ventricular ejection fraction(LVEF)and other cardiac function indicators and SF-36 quality of life scores,as well as atrial fibrillation and asymptomatic atrial fibrillation influences.Results Before treatment,the levels of EDVI,ESVI and LVEF were(41.52±5.68)mL/m2,(20.56±2.73)mL/m2,(53.04±8.10)mL/m2,respectively.After treatment,EDVI,ESVI and LVEF levels were respectively determined(50.36±7.49)mL/m2,(29.78±3.48)mL/m2,(62.15±9.74)mL/m2,the difference was statistically significant after treatment(t=6.781,15.032,5.186,P<0.05).Before treatment,the rate of atrial fibrillation and asymptomatic atrial fibrillation were 3.85%and 3.85%,respectively.After treatment,the rate of atrial fibrillation and asymptomatic atrial fibrillation were 7.70%and 5.77%,respectively.After treatment,they were higher than before treatment.Not statistically significant(χ2=0.078,0.210,P>0.05).Before treatment,the scores of physiological function,physical pain,emotional function,and social function were(51.26±7.79)points,(49.20±7.02)points,(54.61±8.02)points,and(52.68±7.84)points,respectively.The scores of function,physical pain,emotional function and social function were(60.37±8.92)points,(58.74±8.19)points,(62.45±9.36)points,(61.52±9.08)points,respectively,and were higher than before treatment.The difference was statistically significant(t=5.547,6.378,4.587,5.324,P<0.05).Conclusion The implantation of permanent artificial cardiac pacemaker under large C-arm X-ray machine is beneficial to the improvement of heart function and improve the quality of life of patients.Atrial fibrillation and asymptomatic atrial fibrillation are rare.
