全身麻醉和椎管内麻醉在髋关节置换术中的价值体会

The Value of General Anesthesia and Spinal Anesthesia in Total Hip Arthroplasty

目的研究髋关节置换术过程中通过全身麻醉与椎管内麻醉的麻醉效果,对两种麻醉方式的应用价值进行分析。方法随机从该院在2016年5月—2018年5月这一期间内,抽取出进行髋关节置换术的80例患者,展开对照研究,将所有患者按照随机化方法进行分组,一共分成两组,观察组(n=40)患者在手术过程中通过椎管内麻醉,对照组(n=40)患者在手术过程中通过全身麻醉,首先对两组患者并发症的发生概率进行比较,包括心动过速情况、术中发生低血压情况,然后对两组患者的手术指标进行比较,分析两种麻醉方法的麻醉价值。结果观察组患者的并发症的发生概率为5.0%(2/40),发生心动过速的患者有1例,发生低血压情况的患者有1例,对照组患者的并发症的发生概率为15.0%(6/40),4例患者发生心动过速情况,2例患者术中发生低血压情况,组间差异有统计学意义(χ^2=15.925,P<0.05);两组患者的术前准备时间差异无统计学意义(P>0.05),观察组患者的手术时间以及术后患者苏醒时间明显短于对照组患者,差异有统计学意义(P<0.05)。结论进行髋关节置换术的患者,在术中通过椎管内麻醉,可以有效地减少手术时间,加快术后的苏醒,且有效地降低了并发症的发生,提高手术安全性。

Objective To study the anesthetic effect of general anesthesia and spinal anesthesia during hip arthroplasty,and to analyze the application value of two anesthesia methods.Methods Random from the hospital in May 2016 to May 2018,during this period,the extract of 80 patients of hip replacement,a controlled study,will all patients according to randomized grouping method,are divided into two groups,the observation group(n=40)patients by spinal canal anesthesia during the operation,the control group(n=40)patients by general anesthesia during the operation,first of all,the probability of occurrence of complications in two groups of patients,including:tachycardia,intraoperative hypotension occurs,then a measure of the two groups of patients with surgery of comparison,analysis of two anesthesia methods of anesthesia.Results The probability of observation group of patients with complications was 5.0%(2/40),tachycardia patients occurred in 1 case,occur hypotension,1 case of patients in the control group of patients with complications probability was 15.0%(6/40),tachycardia occurred in 4 patients,2 patients intraoperative hypotension occurs,the difference between the two groups was statistically significant(χ^2=15.925,P<0.05);there was no statistically significant difference in the preoperative preparation time between the two groups(P>0.05)and the operation time and postoperative recovery time of the observation group were significantly shorter than that of the control group.There was a statistically significant difference between the two groups(P<0.05).Conclusion In patients undergoing hip replacement,intraoperative spinal anesthesia can effectively reduce the operation time,accelerate postoperative recovery,effectively reduce the occurrence of complications,and improve the safety of surgery,which is worthy of promotion.
