可视喉镜与直接喉镜在困难气道患者中应用效果的优势比较

Comparison of Advantages of Visual Laryngoscope and Direct Laryngoscope in Patients with Difficult Airways

目的可视喉镜与直接喉镜在困难气道患者中的应用效果比较。方法选取该院2017年3月—2019年6月收治的240例困难气道患者为对象,分对照组、观察组,对照组直接喉镜;观察组可视喉镜。结果观察组Ⅰ、Ⅱ级声门暴露级别比例高于对照组,差异有统计学意义(Z=-33.859,P=0.000<0.05)。观察组声门暴露时间、气管插管时间少于对照组,差异有统计学意义(t=9.157、14.931,P=<0.05)。观察组1次性插管成功率95.83%,对照组83.33%,差异有统计学意义(χ^2=8.370,P=0.004<0.05)。观察组并发症发生率1.67%,对照组7.5%,差异有统计学意义(χ^2=3.885,P=0.004<0.05)。结论可视喉镜与直接喉镜相比在困难气道患者中的应用优势是声门暴露范围大,暴露及插管时间少,成功率高,并发症低。

Objective To compare the effects of visual laryngoscope and direct laryngoscope in patients with difficult airways.Methods A total of 240 patients with difficult airway who were treated in the hospital from March 2017 to June 2019 were selected as subjects.They were divided into control group,observation group,and control group with direct laryngoscope;observation group with laryngoscope.Results The proportions of glottic exposure levelⅠandⅡin the observation group were higher than those in the control group,the difference was statistically significant(Z=-33.859,P=0.000<0.05).The glottic exposure time and tracheal intubation time in the observation group were less than those in the control group,and the differences were statistically significant(t=9.157,14.931,P=0.001 P<0.05).The success rate of one-time intubation in the observation group was 95.83%,and the control group was 83.33%.The difference was statistically significant(χ^2=8.370,P=0.004<0.05).The complication rate was 1.67%in the observation group and 7.5%in the control group.The difference was statistically significant(χ^2=3.885,P=0.004<0.05).Conclusion Compared with direct laryngoscope,the advantages of visual laryngoscope in patients with difficult airways are large range of glottic exposure,less exposure and intubation time,high success rate and low complications.
