腋下小切口与胸腔镜治疗79例肺大泡患者术后疼痛与效果比较

Comparison of Postoperative Pain and Effects of Small Axillary Incision and Thoracoscopy in 79 Patients with Bullae

目的针对肺大泡患者实施腋下小切口与胸腔镜联合治疗的临床效果进行分析和研究。方法选择该院收治的79例肺大泡患者作为该次研究的纳入对象,选取时间段为2016年1月—2019年6月期间,按照患者自愿将其分为观察组(39例)和对照组(40例)。采用腋下小切口的方式对对照组予以治疗,采用腋下小切口与胸腔镜联合的方式对观察组予以治疗,对两组患者的治疗效果与术后疼痛情况进行比较。结果对照组的VAS评分为(4.6±1.7)分,引流时间为(6.8±2.3)h,下床活动时间为(4.0±1.7)d,住院时间为(15.5±3.2)d,观察组的VAS评分为(2.5±1.4)分,引流时间为(3.6±1.4)h,下床活动时间为(2.1±1.3)d,住院时间为(9.6±2.1)d。在VAS评分和术后恢复指标方面,观察组明显低于对照组(t=35.624、37.831、22.539、36.825,P<0.05)。对照组的并发症发生率达到了25.0%,观察组的并发症发生率达到了7.7%。在并发症发生率方面,观察组明显低于对照组(χ^2=6.067,P<0.05)。结论针对肺大泡患者实施腋下小切口与胸腔镜联合治疗具有显著的效果,可以有效促进患者的术后恢复,降低患者的术后疼痛感,且安全性更高。

Objective To analyze and study the clinical effects of small incision in the treatment of pulmonary bullae and thoracoscopic surgery.Methods Seventy-seven patients with pulmonary large vesicles admitted to our hospital were enrolled as subjects of this study.The time period was from January 2016 to June 2019,and patients were voluntarily divided into observation groups(39 cases)and control group(40 cases).The control group was treated with a small incision under the armpit.The observation group was treated with a small incision and a thoracoscopic approach.The treatment effect of the two groups was compared with the postoperative pain.Results The VAS score of the control group was(4.6±1.7)points,the drainage time was(6.8±2.3)h,the time of getting out of bed was(4.0±1.7)d,and the hospitalization time was(15.5±3.2)d.The VAS of the observation group.The score was(2.5±1.4)points,the drainage time was(3.6±1.4)h,the time of getting out of bed was(2.1±1.3)d,and the hospitalization time was(9.6±2.1)d.In the VAS score and postoperative recovery index,the observation group was significantly lower than the control group(t=35.624,37.831,22.539,36.825,P<0.05).The complication rate of the control group reached 25.0%,and the complication rate of the observation group reached 7.7%.In terms of the incidence of complications,the observation group was significantly lower than the control group(χ^2=6.067,P<0.05).Conclusion A small incision in the treatment of pulmonary bullae with thoracoscopic surgery has a significant effect,can effectively promote postoperative recovery,reduce postoperative pain,and is safer.
