超声乳化白内障吸除、人工晶状体植入联合小梁切除术治疗白内障合并青光眼的疗效分析

Analysis of Phacoemulsification Cataract Extraction, Intraocular Lens Implantation and Trabeculectomy in the Treatment of Cataract with Glaucoma

目的在进行白内障合并青光眼治疗时,对超声乳化白内障吸除术、人工晶状体植入联合小梁切除术的治疗效果进行探究。方法回顾性分析在该院2016年4月-2018年10月期间治疗的白内障合并青光眼196例患者的临床资料,分为研究A组(n=98)和研究B组(n=98),研究A组进行超声乳化白内障吸除、人工晶状体植入联合小梁切除术治疗,研究B组进行囊外白内障摘除、人工晶状体植入联合小梁切除术治疗。对比两组的眼科相关参数情况、并发症情况、术后六月生活质量情况。结果研究A组患者的IOP、PAS数据明显低于研究B组,BCVA、CAC、房角、AOD500等数据高于研究B组,两组数据对比差异有统计学意义(χ^2=7.422,P<0.01)。研究A组总发生并发症概率9.18%,研究B组总发生并发症概率21.42%,研究A组数据明显低于研究B组,两组数据对比差异有统计学意义(χ^2=9.859,P<0.01)。研究A组患者躯体功能、社会功能、总体健康评分均高于研究B组,两组数据对比差异有统计学意义(χ^2=7.251,P<0.01)。结论在治疗白内障合并青光眼时,超声乳化白内障吸除术、人工晶状体植入联合小梁切除术治疗效果更明显,患者各项眼科的相关参数水平较好、并发症发生概率较小、患者远期生活治疗较高,可以在临床上大力推广。

Objective To investigate the effects of phacoemulsification cataract extraction,intraocular lens implantation and trabeculectomy in the treatment of cataract combined with glaucoma.Methods The clinical data of 196 patients with cataract and glaucoma treated in the hospital from April 2016 to October 2018 were retrospectively analyzed.They were divided into study group A(n=98)and study group B(n=98).Group A was treated with phacoemulsification cataract extraction,intraocular lens implantation and trabeculectomy.Study Group B was performed with extracapsular cataract extraction,intraocular lens implantation and trabeculectomy.The ophthalmological parameters,complications,and quality of life were compared between the two groups after 6 months of surgery.Results The IOP and PAS data of patients in study group A were significantly lower than those in study group B,and the data of BCVA,CAC,room angle,and AOD500 were higher than those in study group B.The difference between the two groups was statistically significant(χ^2=7.422,P<0.01).The total complication probability in study group A was 9.18%,and the total complication probability in study group B was 21.42%.The data in study group A was significantly lower than that in study group B.The difference between the two groups was statistically significant(χ^2=9.859,P<0.01).The physical function,social function,and overall health scores of patients in study group A were higher than those in study group B.The difference between the two groups was statistically significant(χ^2=7.251,P<0.01).Conclusion In the treatment of cataract combined with glaucoma,phacoemulsification cataract extraction,intraocular lens implantation and trabeculectomy are more effective.The patient’s various ophthalmological parameters are better,the complications are less likely,and the patient’s long-term life therapy is high and can be promoted vigorously in the clinic.
