综合护理干预对甲状腺手术患者术后疼痛及不良情绪的影响

Effect of Comprehensive Nursing Intervention on Postoperative Pain and Emotions in Patients with Thyroid Surgery

目的研究甲状腺手术患者术后实施综合护理干预对疼通以及不良情绪产生的临床效果。方法在该院选取86例行甲状腺手术患者,时间选为2017年6月-2018年6月,使用抽签法进行分组,分为两组,实验组与对照组,各43例。实施一般护理为对照组,实施综合护理干预为实验组,对两组护理效果进行分析,使用抑郁自评量表(SDS)与焦虑自评量表(SAS)对患者的不良情绪进行评分,对比两组护理前后疼痛程度。结果对两组SDS、SAS评分进行分析,护理前,实验组SDS、SAS评分分别为(72.31±11.20)分、(73.52±11.22)分,对照组分别为(72.03±11.33)分、(73.26±11.31)分,两组差异无统计学意义(t=0.115、0.107,P>0.05),护理后,两组均有所改善,实验组SDS、SAS评分分别为(41.74±6.38)分、(37.53±5.71)分,对照组分别为(51.23±7.88)分、(49.63±7.36)分,但实验组要更佳于对照组,差异有统计学意义(t=6.137、8.154,P<0.05);对两组护理前后VAS、NRS评分分析,护理前,实验组VAS、NRS评分分别为(7.32±1.77)分、(6.52±1.21)分,对照组分别为(7.54±1.52)分、(6.47±1.36)分,两组数据差异无统计学意义(t=0.618、0.108,P>0.05),护理后,实验组VAS、NRS评分分别为(3.52±0.89)分、(3.12±0.68)分,对照组分别为(5.21±1.22)分、(4.36±1.07)分,两组患者疼痛均有所改善,但实验组要优于对照组,差异有统计学意义(t=7.338、6.414,P<0.05);分析两组患者并发症发生率,实验组6.98%低于对照组25.58%(χ^2=12.691,P<0.05),对比两组护理后的总有效率,实验组97.67%高于对照组83.72%,差异有统计学意义(χ^2=11.530,P<0.05)。结论对甲状腺手术患者实施综合护理干预有良好的临床效果,可以减轻患者疼痛程度,患者其负面情绪,使其可以以更好的状态面对治疗,促进治疗进展,使患者可以加速恢复,值得临床推广与应用。

Objective To study the clinical effects of comprehensive nursing intervention on pain and bad mood in patients with thyroid surgery.Methods Eighty-six patients undergoing thyroid surgery were selected in the hospital from June 2017 to June 2018.They were divided into two groups using the lottery method,and the experimental group and the control group,each with 43 cases.The general care was used as the control group,and the comprehensive nursing intervention was used as the experimental group.The nursing effects of the two groups were analyzed.The selfdepression scale(SDS)and the self-rating anxiety scale(SAS)were used to score the patients’adverse emotions degree of pain before and after nursing in the two groups.Results The SDS and SAS scores of the two groups were analyzed.Before nursing,the SDS and SAS scores of the experimental group were(72.31±11.20)points and(73.52±11.22)points,and the control group were(72.03±11.33)points and(73.26±11.31)points.The difference was not statistically significant between the two groups(t=0.115,0.107,P>0.05).After nursing,both groups improved.The SDS and SAS scores of the experimental group were(41.74±6.38)points and(37.53±5.71)points,and the control group was(51.23±7.88)points,(49.63±7.36)points,but the experimental group was better than the control group,the difference was statistically significant(t=6.137,8.154,P<0.05);analysis of VAS and NRS scores before and after the two groups of care,before nursing,the VAS and NRS scores of the experimental group were(7.32±1.77)points and(6.52±1.21)points,and the control group was(7.54±1.52)points and(6.47±1.36)points.The difference was not statistically significant between the two groups(t=0.618,0.108,P>0.05).After nursing,the VAS and NRS scores of the experimental group were(3.52±0.89)points and(3.12±0.68)points,and the control group were(5.21±1.22)points and(4.36±1.07)points.The pain inboth groups was improved,but the experimental group was better than the control group,the difference was statistically significant(t=7.338,6.414,P<0.05);analyze the complications of the two groups of patients:the birth rate of the experimental group was 6.98%lower than the control group of 25.58%(χ^2=12.691,P<0.05).Comparing the total effective rate of the two groups after nursing,the experimental group was 97.67%higher than the control group of83.72%,and the difference was statistically significant(χ^2=11.530,P<0.05).Conclusion The comprehensive nursing intervention for patients with thyroid surgery has good clinical effects,which can reduce the pain of patients and their negative emotions,so that they can face the treatment in a better state,promote the progress of treatment,and enable patients to accelerate recovery.It is worthy of clinical promotion and application.
