全麻诱导期右美托咪定丙泊酚干预对气管插管过程的影响 被引量:1

摘要 目的探讨全麻诱导期右美托咪定丙泊酚干预对气管插管过程的影响。方法选取该院2017年1月—2018年12月接诊的气管插管患者40例进行研究,将所有患者按照电脑随机表法平均分成观察组20例和对照组20例,对照组患者静脉注射丙泊酚、咪达唑仑、芬太尼及顺势阿曲库铵治疗,肌松达标后进行气管插管;观察组患者在麻醉诱导前使用微泵注射右美托咪定,然后按照对照组的麻醉药物使用顺序进行麻醉。对比两组患者全麻诱导后、气管插管后5 min以及气管插管后10 min的血压和心率水平。结果两组患者全麻诱导后收缩压、舒张压和心率对比差异无统计学意义(P>0.05);气管插管后5 min、气管插管后10 min后观察组患者的收缩压(129.58±4.42)mmHg、(124.71±3.13)mmHg明显高于对照组(115.22±5.31)mmHg、(110.42±4.06)mmHg(t=9.295、12.466,P<0.05);观察组舒张压明显明显低于对照组;心率水平均高于对照组,差异有统计学意义(P<0.05)。结论对于进行气管插管麻醉患者在全麻诱导期使用右美托咪定联合丙泊酚能够提高麻醉效果,可减少机体的应激反应,对于稳定生命体征有重要作用,值得推广应用。 Objective To investigate the effect of dexmedetomidine and propofol intervention on tracheal intubation during the induction period of general anesthesia.Methods Forty patients with tracheal intubation admitted in thehospital from January 2017 to December 2018 were selected for study.All patients were equally divided into observation group 20 and control group 20 according to the computerized random table method.After treatment with propofol,midazolam,fentanyl,and homeopathic atracurium,tracheal intubation was performed after muscle relaxation was achieved.Patients in the observation group were injected with dexmedetomidine using a micropump before induction of anesthesia.The anesthetic drug was used in sequence for anesthesia.The blood pressure and heart rate of the two groups were compared after induction of general anesthesia,5 minutes after tracheal intubation,and 10 minutes after tracheal intubation.Results There was no statistically significant difference in systolic blood pressure,diastolic blood pressure,and heart rate between the two groups after general anesthesia induction(P>0.05);5 min after tracheal intubation and 10 min after tracheal intubation(124.71±3.13)mmHg was significantly higher than the control group(115.22±5.31)mmHg,(110.42±4.06)mmHg(t=9.295,12.466,P<0.05);the diastolic blood pressure in the observation group was significantly lower than the control group;heart rateis higher than cortrol,the difference was statistically significant(P<0.05).Conclusion For patients undergoing tracheal intubation anesthesia,the use of dexmedetomidine combined with propofol during the induction period of general anesthesia can improve the anesthesia effect,reduce the body's stress response,and have an important role in stabilizing vital signs,which is worthy of popularization and application.
