双侧解剖型锁定钢板内固定治疗成人肱骨髁上骨折效果探讨

Effect of Bilateral Anatomical Locking Plate Internal Fixation on Adult Humeral Supracondylar Fracture

目的以成人肱骨髁上骨折患者为例,对双侧解剖型锁定钢板内固定治疗方法在其中的应用效果进行分析评估。方法该次以该院在2017年2月—2018年1月收治的86例成人肱骨髁上骨折患者作为研究的对象,按随机的方式分成两个不同的组别,每组平均为43例;其中,对照组患者采取普通钢板内固定术治疗,观察组患者则采取双侧解剖型锁定钢板内固定术治疗,进一步对两组手术治疗效果进行分析评价。结果①在骨折治疗总有效率方面,观察组为95.35%,和对照组的65.12%比较显著更高,两组之间的数据差异有统计学意义(χ^2=10.263,P<0.05)。②在手术时间及住院时间方面,观察组分别为(116.54±12.76)min、(15.43±2.11)d,均明显短于对照组的(139.27±12.45)min、(19.87±2.13)d,两组之间的数据差异有统计学意义(t=8.273、6.521,P<0.05)。③观察组,术后情感职能为(86.28±1.34)分、精神健康为(88.29±1.45)分、生理职能为(88.45±1.53)分、社会功能为(87.19±1.56)分、精力为(81.27±1.54)分、躯体疼痛为(83.27±1.20)分;对照组情感职能为(65.91±1.53)分、精神健康为(66.26±1.56)分、生理职能为(67.29±1.32)分、社会功能为(66.59±1.28)分、精力为(65.10±1.29)分、躯体疼痛为(62.19±1.18)分。根据数据可知,在情感职能、精神健康、生理职能、社会功能、精力以及躯体疼痛6项生活质量指标评分方面,观察组术后均明显高于对照组高,两组之间的数据差异有统计学意义(t=10.286、10.359、10.428、10.290、10.327、10.215,P<0.05)。④观察组术后并发症总发生率为4.65%,明显要比对照组的20.93%低,两组之间的数据差异有统计学意义(χ^2=9.286,P<0.05)。结论在临床手术过程中,针对成人肱骨髁上骨折患者,采取双侧解剖型锁定钢板内固定术治疗具备显著的价值,可缩短手术时间及住院时间,改善患者生活质量,降低术后并发症发生率;因此,具备在临床手术中采纳及应用的价值。

Objective To analyze and evaluate the application of bilateral anatomical locking plate internal fixation in adult patients with supracondylar fracture of humerus.Methods In this study,86 adult patients with supracondylar fractures of the humerus who were treated in the hospital from February 2017 to January 2018 were taken as the research object.They were randomly divided into two different groups,with an average of 43 cases in each group;Among them,the patients in the control group were treated with ordinary steel plate internal fixation,and the patients in the observation group were treated with bilateral anatomical locking steel plate internal fixation.The surgical treatment effects of the two groups were further analyzed and evaluated.Results 1.The total effective rate of fracture treatment was 95.35%in the observation group and 65.12%in the control group.The data between the two groups was significantly different(χ^2=10.263,P<0.05).2.In terms of operation time and hospitalization time,the observation group was(116.54±12.76)min and(15.43±2.11)d,which were significantly shorter than the control group's(139.27±12.45)min and(19.87±2.13)d.There were significant differences in the data between the two groups(t=8.273,6.521,P<0.05).3.In the observation group,postoperative emotional function was(86.28±1.34)points,mental health was(88.29±1.45)points,physiological function was(88.45±1.53)points,social function was(87.19±1.56)points,and energy was(81.27±1.54)points,physical pain was(83.27±1.20)points;control group was(65.91±1.53)points for emotional functions,(66.26±1.56)points for mental health,(67.29±1.32)points for physiological functions,social function was(66.59±1.28)points,energy was(65.10±1.29)points,and physical pain was(62.19±1.18)points.According to the data,in the six quality of life scores of emotional function,mental health,physiological function,social function,energy and physical pain,the observation group was significantly higher than the control group after operation,and the data between the two groups were significantly different(t=10.286,10.359,10.428,10.290,10.327,10.215,P<0.05).4.The total incidence of postoperative complications in the observation group was 4.65%,which was significantly lower than the 20.93%in the control group.There was a significant difference in the data between the two groups(χ^2=9.286,P<0.05).Conclusion During clinical surgery,it is of great value to treat bilateral adult humeral supracondylar fractures with bilateral anatomical locking plate internal fixation,which can shorten the operation time and hospitalization time,improve the quality of life of patients,and reduce postoperative complications rate;therefore,it has value for adoption and application in clinical surgery.
