摘要
目的对新生儿诊断窒息多器官衰竭应用新生儿脐动脉血的价值进行分析探讨。方法选取该院2017年1月—2019年3月期间治疗的120例窒息新生儿作为研究对象,对其进行病情回顾分析。将其Apgar评分、BE检测阳性器官衰竭标准和脐动脉血血气分析ph值进行对比。结果据研究发现,接诊治疗的患儿中存在多器官衰竭损伤的并发几率为(58.30%),如以新生儿分娩后Apgar评分≤4/6分为诊断标准,则多器官损害阳性检测值为(45.12%)、器官敏感度评分为(30.16%)、器官特异度评分为(24.56%),如以PH值<6或BE值≤-15 mmol/L为诊断标准,则多器官损害阳性预测率为(70.20%)、器官敏感度为(63.15%)、器官特异度为(73.11%)。结论对新生儿动脉血气进行pH值与BE值监测,能够有效提升新生儿窒息评价与多器官损害标准,如将Apgar评分与临床诊断结果相结合,能够使其诊断准确性得以提升。
Objective To analyze the value of neonatal umbilical arterial blood in the diagnosis of asphyxia in multiple organ failure.Methods A total of 120 asphyxiated neonates treated in the hospital from January 2017 to March 2019 were selected as the research subjects,and their condition was retrospectively analyzed.Compare their Apgar score,BE-positive organ failure criteria and umbilical arterial blood gas analysis pH.Results According to the research,the incidence of multiple organ failure injury in the children receiving treatment was(58.30%).If the Apgar score≤4/6 is divided into diagnostic criteria after the delivery of the newborn,the positive value of multiple organ damage was detected(45.12%),organ sensitivity score(30.16%),and organ specificity score(24.56%).If the pH value was less than 6 or the BE value was less than or equal to-15 mmol/L as the diagnostic criteria,then multiple organ damage was positively prediction rate was(70.20%),the organ sensitivity was(63.15%),and the organ specificity was(73.11%).Conclusion The monitoring of ph and BE values of neonatal arterial blood gas can effectively improve the criteria for neonatal asphyxia and multi-organ damage.For example,the combination of Apgar score and clinical diagnosis results can improve the diagnostic accuracy.
