分析中晚期宫颈癌调强放疗与三维适型放疗联合后装治疗的近期疗效

Analysis of Short-term Efficacy of Intensity-modulated Radiotherapy and Three-dimensional Conformal Radiotherapy Combined with Post-packaging Therapy for Advanced Cervical Cancer

目的观察分析中晚期宫颈癌调强放疗是否比三维适型放疗在近期疗效、急慢性放疗不良反应和危及器官的剂量学上更具有优势。方法选择2012年6月—2014年6月接受的符合要求的患者为研究对象,随机进行分析,IMRT组和3D-CRT组分别采取5野IMRT和4野箱式放疗,两组均结合后装腔内放疗及每周同步顺铂化疗。结果分析不同程度的放射性直肠炎、放射性膀胱炎发生几率,IMRT组0度和1度的几率高于3D-CRT组,差异有统计学意义(P<0.05)。IMRT组的总有效率高于3D-CRT组,分别是95.0%和88.6%(χ^2=6.59,P=0.004)。结论与3D-CRT对比,IMRT在治疗中晚期宫颈癌中能够降低肠道的急慢性放射性损伤和血液学损伤,完全缓解率优于3D-CRT。

Objective To observe and analyze whether intensity-modulated radiotherapy for advanced cervical cancer is more advantageous than three-dimensional conformal radiotherapy in terms of short-term efficacy,adverse reactions of acute and chronic radiotherapy,and dosimetry that endangers organs.Methods Patients who met the requirements from June 2012 to June 2014 were selected as the research subjects and analyzed randomly.The IMRT group and the 3D-CRT group were treated with 5-field IMRT and 4-field box radiotherapy,respectively.Intracavity radiotherapy and weekly cisplatin chemotherapy were adopted.Results After analyzing the incidence of radiation proctitis and cystitis in different degrees,the incidence of 0 degree and 1 degree in IMRT group was higher than that in 3d-crt group,and the difference was statistically significant(P<0.05).The total efficiency of the IMRT group was 95.0%and 88.6%higher than that of the 3d-crt group(χ^2=6.59,P=0.004).Conclusion Compared with 3D-CRT,IMRT can reduce the acute and chronic radiation damage and hematological damage of the intestine in the treatment of advanced cervical cancer.The complete remission rate is better than 3D-CRT.
