摘要
目的探究右美托咪定不同剂量调节对麻醉维持自主呼吸恢复时间、定向力恢复时间以及不良反应发生率的影响作用。方法择选该院2018年6月—2019年6月,接收的90例腹腔镜全麻手术患者为研究对象,按照计算机随机方法,均分为3组。A、B、C 3组均给予右美托咪定,麻醉剂量由高到低,分别是1.00μg/kg、0.50μg/kg和0.25μg/kg。分别记录3组患者的自主呼吸恢复时间、定向力恢复时间、苏醒时间和拔管时间。统计3组患者术后不良反应发生情况,并实施对比。结果3组患者中,A组患者无论是自主呼吸恢复时间(7.82±0.87)min、定向力恢复时间(10.02±2.03)min、苏醒时间(13.44±1.71)min、拔管时间(17.75±1.72)min,均要短于B组的(8.64±0.42)min、(21.92±2.57)min、(14.23±1.62)min、(18.12±1.16)min、C组的(10.31±0.73)min、(22.73±2.85)min、(16.11±1.72)min、(19.43±2.04)min,同时B组要短于C组,差异有统计学意义(P<0.05);不良反应统计对比结果显示,C组患者的不良反应发生率最高43.3%,其次是A组20.0%,最后是C组10%,组间差异统计学有意义(P<0.05)。结论腹腔镜全麻手术患者行右美托咪定,当剂量为1.00μg/kg、0.50μg/kg时,拥有较好的麻醉效果,但考虑到不良反应发生情况,推荐采取0.50μg/kg的剂量方案,以提高麻醉安全性。
Objective To explore the effects of different dose adjustments of dexmedetomidine on anesthesia maintenance spontaneous breathing recovery time,directional force recovery time the incidence of adverse reactions.Methods 90 patients undergoing laparoscopic general anesthesia were selected from our hospital from June 2018 to June 2019.They were divided into 3 groups according to the random computer method.Dexmedetomidine was given to groups A,B,and C.The anesthesia doses were from high to low,which were 1.00μg/kg,0.50μg/kg,and 0.25μg/kg,respectively.The spontaneous breathing recovery time,directional force recovery time,awake time,and extubation time of the three groups of patients were recorded separately.The incidence of adverse reactions in the three groups of patients was counted and compared.Results Among the three groups of patients,patients in group A were required to have spontaneous breathing recovery time(7.82±0.87)min,directional force recovery time(10.02±2.03)min,wake-up time(13.44±1.71)min,and extubation time(17.75±1.72)min,shorter than group B of(8.64±0.42)min,(21.92±2.57)min,(14.23±1.62),(18.12±1.16)min,and group C of(10.31±0.73)min,(22.73±2.85)min,(16.11±1.72)min,(19.43±2.04)min,and shorter than group C Statistical significance(P<0.05);The statistical comparison of adverse reactions showed that the incidence of adverse reactions was highest in group C,43.3%,followed by group A by 20.0%,and finally group C by 10.0%.The difference between groups was statistically significant(P<0.05).Conclusion Dexmedetomidine was performed in patients undergoing laparoscopic general anesthesia.When the dosage was 1.00μg/kg,0.50μg/kg,it had a good anesthetic effect.However,considering the occurrence of adverse reactions,it is recommended to take 0.50μg/kg dose regimen to improve anesthesia safety.
