人工流产术前应用卡前列甲酯栓的临床分析

Clinical Analysis of Carboprost Methyl Ester Suppository before Artificial Abortion

目的综合分析人工流产术前应用卡前列甲酯栓对手术效果和用药安全性的影响。方法选取2017年3月-2019年6月于该院妇产科行人工流产术的产妇56例,采用随机数表法分为两组,每组28例,观察组产妇在术前2 h置1 mg卡前列甲酯栓片于宫颈口,对照组产妇术前不使用任何药物,两组手术人员、操作及流程相同,观察并记录两组的手术指标、宫颈扩张难易度和手术不良反应情况,并加以比较。结果①观察组产妇术中出血量、手术时间及术后24 h疼痛VAS评分分别为(8.4±1.2)mL、(5.7±1.1)min、(1.6±0.5)分均少于对照组,差异有统计学意义(t=23.503、18.129、12.195,P=0.000 <0.05);②观察组产妇扩宫度(宫颈扩张难易度)评分为(2.5±0.4)分,显著优于对照组(1.6±0.5)分,(t=7.437,P=0.000 <0.05);③观察组不良反应率为10.7%,与对照组3.6%相比差异无统计学意义(χ^2=0.269,P=0.604 P>0.05)。结论在人工流产术前应用卡前列甲酯栓,有助于软化宫颈口、减小扩宫难度、提高手术效果,且不良反应少,用药安全,值得临床推广应用。

Objective To comprehensively analyze the effects of preoperative carboprost methyl ester suppository on surgical effect and drug safety. Methods 56 women who underwent artificial abortion in the obstetrics and gynecology department of this hospital from March 2017 to June 2019 were selected and divided into two groups using a random number table method, with 28 cases in each group. 1 mg carboprost methyl ester suppository was used in the cervix.The control group did not use any drugs before surgery. The surgical staff, procedures and procedures of the two groups were the same. Observation and recording of surgical indicators, easiness of cervical dilatation and adverse reactions of the two groups were compared. Results ①In the observation group, the intraoperative blood loss, operative time and 24 h postoperative pain VAS scores were(8.4 ± 1.2) mL,(5.7 ± 1.1) min, and(1.6 ± 0.5) points, which were less than those in the control group, and the difference was statistically significant(t=23.503, 18.129, 12.195, P=0.000 P<0.05);②maternal dilatation in the observation group(difficulty of cervical dilatation) score was(2.5 ± 0.4) points, which was significantly better than the control group(1.6 ± 0.5) points(t=7.437, P=0.000 <0.05);③ the adverse reaction rate in the observation group was 10.7%, compared with 3.6% of the control group, the difference was not statistically significant(χ^2=0.269, P=0.604 >0.05). Conclusion Carboprost methyl ester suppository before artificial abortion can help soften the cervix, reduce the difficulty of expanding the uterus, improve the surgical effect, and has fewer adverse reactions and safe medication, which is worthy of clinical application.
