急性脑梗死患者血浆脑利钠肽变化与左心室功能分析

摘要 目的研究急性脑梗死患者血浆脑利钠肽的变化和左心室功能的改变情况,供临床借鉴。方法方便选取2017年5月—2019年7月该院接诊且发病时间为48 h内的急性脑梗死患者30例为试验组,同时以该院同期接诊的健康体检者30名作为对照组。两组都接受超声心动图检查,并采血检测血浆脑利钠肽水平。综合分析两组的各指标检测的结果,同时对其作出比较。结果试验组的血浆脑利钠肽水平为(512.87±80.65)pg/mL,比对照组的(149.64±76.32)pg/mL高(t=13.462,P<0.05)。试验组的室间隔厚度为(10.74±2.36)mm,比对照组的(9.18±1.24)mm大(t=3.005,P<0.05)。试验组的A峰最大速度为(73.18±15.92)cm/s,比对照组的(57.94±12.36)cm/s高(t=4.822,P<0.05)。试验组的每博输出量为(39.02±13.11)mL、E峰最大速度为(65.29±15.16)cm/s、心输出量为(3.07±1.42)L/min、E峰和A峰最大速度比值为(0.97±0.29)、左室射血分数为(44.95±14.83)%,比对照组的(54.28±12.64)mL、(83.94±16.15)cm/s、(3.93±1.31)L/min、(1.48±0.23)以及(66.02±10.74)%低(t=4.731、4.032、3.152、3.221、4.971,P<0.05)。结论对于急性脑梗死患者而言,其在发病后会出现血浆脑利钠肽水平明显增高的情况。并且,急性脑梗死的发生还能导致左心室舒张与收缩功能的降低,这和急性脑梗死的病理生理机制有着较为密切的关系,需引起临床的重视。 Objective To study the changes of plasma brain natriuretic peptide and left ventricular function in patients with acute cerebral infarction,so as to provide clinical reference.Methods 30 patients with acute cerebral infarction who were admitted to the hospital from May 2017 to July 2019 and the onset time was within 48h were convenient selection taken as the experimental group,and 30 patients who underwent health examination during the same period were taken as the control group.Both groups were examined by echocardiography and blood samples were collected to detect the plasma level of brain natriuretic peptide.Comprehensive analysis of the two groups of indicators test results,and make a comparison.Results The plasma level of brain natriuretic peptide in the test group was(512.87±80.65)pg/mL,higher than that in the control group(149.64±76.32)pg/mL(t=13.462,P<0.05).The ventricular septal thickness of the experimental group was(10.74±2.36)mm,which was larger than that of the control group(9.18±1.24)mm(t=3.005,P<0.05).The maximum peak velocity of the test group was(73.18±15.92)cm/s,which was higher than that of the control group(57.94±12.36)cm/s(t=4.822,P<0.05).In the experimental group,the output per wave was(39.02±13.11)mL,the maximum velocity of peak E was(65.29±15.16)cm/s,the cardiac output was(3.07±1.42)L/min,the maximum velocity ratio of peak E and peak A was(0.97±0.29),and the left ventricular ejection fraction was(44.95±14.83)%.Compared with control group(54.28±12.64)mL,(83.94±16.15)cm/s,(3.93±1.31)L/min,(1.48±0.23)and(66.02±10.74)%(t=4.731,4.032,3.152,3.221,4.971,P<0.05).Conclusion For patients with acute cerebral infarction,the plasma natriuretic peptide level will increase significantly after onset.In addition,the occurrence of acute cerebral infarction can also lead to the reduction of left ventricular diastolic and systolic functions,which is closely related to the pathophysiological mechanism of acute cerebral infarction,which needs to be paid attention to clinically.
