血CEA、NSE、CYFRA21-1和SCC联合检测在肺癌诊断中的应用研究

目的研究肺癌诊断中联合检测血CEA、NSE、CYFRA21-1和SCC指标水平的价值。方法方便选取2016年1月—2019年4月该院80例患者及40名健康体检者为对象,将其中40例肺癌患者视作肺癌组,40例肺良性疾病患者视作肺良性疾病组,并以同期40名健康体检者为对照组,3组研究对象均接受4项指标联合检测,分析结果。结果小细胞肺癌血CEA、、NSECYFRA21-1、SCC水平分别为(4.75±1.83)ng/mL、(34.76±20.46)ng/mL、(4.05±2.31)ng/mL、(1.69±1.13)ng/mL,与非小细胞肺癌(9.62±2.28)ng/mL、(10.55±7.21)ng/mL、(7.49±3.62)ng/mL、(10.72±3.35)ng/mL差异有统计学意义(t=3.65、4.87、2.96、8.75,P<0.05);肺癌、肺良性疾病、健康体检者各指标水平两两之间差异有统计学意义(P<0.05);单一指标诊断的敏感度和准确度较联合诊断更低(P<0.05),但特异度与联合诊断差异无统计学意义(P>0.05)。结论4项指标联合检测可以辅助诊断肺癌,为临床诊疗方案的制订提供有用信息。

Objective To study the value of combined detection of CEA,NSE,CYFRA21-1 and SCC in the diagnosis of lung cancer.Methods From January 2016 to April 2019,80 patients with lung cancer and 40 patients with healthy physical examination were convenient selected as subjects.40 patients with lung cancer and 40 patients with benign lung diseases were regarded as benign lung diseases group,and 40 patients with healthy physical examination at the same time were taken as control group,and the results were analyzed.Results The levels of CEA,NSE,SCC and CYFRA21-1 in small cell lung cancer are(4.75±1.83)ng/mL,(34.76±20.46)ng/mL,(4.05±2.31)ng/mL,(1.69±1.13)ng/mL,significantly different from non-small cell lung cancer(9.62±2.28)ng/mL,(10.55±7.21)ng/mL,(7.49±3.62)ng/mL,(10.72±3.35)ng/mL,the difference was statistically significant(t=3.65,4.87,2.96,8.75,P<0.05);there is a significant difference between the levels of each index of lung cancer,benign lung disease,and health checkups(P<0.05);the sensitivity and accuracy of the single index diagnosis are lower than the combined diagnosis(P<0.05),but the specificity was not significantly different from the combined diagnosis(P>0.05).Conclusion The combined detection of four indexes can assist in the diagnosis of lung cancer and provide useful information for the development of the clinical diagnosis and treatment program.
