摘要
目的分析探讨纤维支气管镜肺泡灌洗对重症肺炎患者的治疗效果。方法便利选择在该院进行治疗的重症肺炎患者66例,选择时间为2017年6月-2019年6月,将其根据数字随机法分为普通组和研究组(均33例),普通组行常规治疗,研究组行纤维支气管镜肺泡灌洗治疗,对比两组患者血气指标、炎性指标及治疗效果。结果研究组患者的PaO2、PaCO2、pH等血气指标改善程度较普通组更为明显,差异有统计学意义(t=7.380,P=0.000;t=5.617,P=0.000;t=2.292,P=0.025)。CRP、PCT等炎性指标水平较普通组明显更低,差异有统计学意义(t=5.755,P=0.000;t=4.012,P=0.000)。治疗有效率较普通组明显更高(93.94%vs 75.76%)(χ~2=4.243,P=0.039)。结论将纤维支气管镜肺泡灌洗应用于重症肺炎患者的治疗,能显著改善患者的血气指标及炎性指标,缩短患者的恢复进程,提高治疗有效率,安全性和有效性明显较高,值得临床推广使用。
Objective To analyze and discuss the therapeutic effect of bronchoscopic alveolar lavage on severe pneumonia.Methods Sixty-six patients with severe pneumonia were convenient selected and treated in the hospital from June 2017 to June 2019. They were divided into normal group and research group(33 cases in each group) according to the digital random method. The normal group was treated with routine treatment, while the research group was treated with bronchoscopic alveolar lavage. The blood gas and inflammatory indexes of the two groups were compared. And the treatment effect. Results The improvement of blood gas indexes such as PaO2, PaCO2 and pH in the study group was more obvious than that in the normal group(t=7.380, P=0.000;t=5.617, P=0.000;t=2.292, P=0.025). The levels of inflammatory markers such as CRP and PCT were significantly lower than those of the normal group(t=5.755, P=0.000;t=4.012, P=0.000). The treatment efficiency was significantly higher than that of the normal group(93.94% vs 75.76)(χ~2=4.243, P =0.039).Conclusion The application of bronchoscopic alveolar lavage in the treatment of severe pneumonia can significantly improve the blood gas index and inflammatory index of patients, shorten the recovery process of patients, improve the treatment efficiency, safety and effectiveness, which is worthy of clinical application.
