摘要 目的探讨二甲双胍对糖尿病患者GLP-1水平的影响。方法方便选取2017年5月-2018年9月重庆三峡中心医院治疗的初发2型糖尿病患者75例作为对象,将患者分为二甲双胍组15例,格列美脲组15例,吡格列酮组15例,瑞格列奈组15例,阿卡波糖组15例。比较5组患者治疗前及治疗后FPG、HOMA-IR、FINS、GLP-1水平变化。结果五组患者治疗前的FPG、HOMA-IR、FINS、GLP-1水平差异无统计学意义(P>0.05);二甲双胍组治疗后的FPG、FINS、GLP-1、HOMA-IR指标均优于治疗前,且优于其他组,其中治疗后二双胍组GLP-1 (22.13±3.52),格列美脲组GLP-1(20.94±3.38),吡格列酮组GLP-1(19.45±1.95),瑞格列奈组GLP-1(21.03±2.13),阿卡波糖组GLP-1(20.84±2.57),二甲双胍组GLP-1水平高于其他4组,差异有统计学意义(F=5.475,P<0.05)。结论二甲双胍具有提高糖尿病患者胰岛β细胞分泌量及功能,降低胰岛素抵抗能力,与GLP-1水平的提高有密切关系。 Objective To investigate the effect of metformin on the level of GLP-1 in diabetic patients. Methods From May2017 to September 2018, 75 cases of newly diagnosed type 2 diabetes patients treated by Chongqing Three Gorges Central Hospital were targeted, the patients were convenient divided into 15 cases in the metformin group, 15 cases in the glimepiride group, 15 cases in the pioglitazone group, 15 cases in the reglitazone group, and 15 cases in the acarbose group.The levels of FPG, HOMA-IR, FINS, and GLP-1 were compared before and after treatment in the five groups. Results The levels of FPG, HOMA-IR, FINS and GLP-1 in the five groups were not significantly different before treatment, and the difference was not statistically significant(P>0.05). The FPG, FINS, GLP-1 and HOMA-IR indicators after treatment with metformin group Both were superior to those before treatment, and were superior to other groups, including GLP-1(22.13±3.52) in the diterpene group, GLP-1(20.94±3.38) in the glimepiride group, and GLP-1 in the pioglitazone group(19.45±1.95). GLP-1(21.03±2.13) in the repaglinide group and GLP-1(20.84±2.57) in the acarbose group. The GLP-1 level in the metformin group was higher than the other four groups, and the difference was statistically significant(F =5.475,P <0.05).Conclusion Metformin can improve the function of islet beta cells and insulin resistance in diabetic patients, which is closely related to the increase of GLP-1 level and the inhibition of HOMA-IR.
