基于临床调查法的阴虚证中医疗效评价量表条目筛选

Items Selection of A Patient Reported Syndrome Scale of Yin Deficiency Syndrome Based on the Clinical Investigation

目的研制阴虚证中医疗效患者自评量表,应用临床调查法多元统计筛选、优化条目。方法初步编制阴虚证中医疗效患者自评量表,现场调查阴虚证患者及健康者,分别从条目的清晰度、天花板地板效应、离散趋势法、相关系数法、克朗巴赫系数法、因子分析法及T检验法等七方面考评条目设置是否合理。结果共纳入受试者80例,其中阴虚证患者60例,正常健康者20例。应用以上统计方法对调查结果进行分析、汇总,“突然一阵脸热”条目同时被2种统计方法选中删除。结合专家意见,认为条目“突然一阵脸热”与“忽上来一股热劲”相似,且后者包含前者,故删除“突然一阵脸热”条目,确定了包含3个维度、10个条目的终选量表。结论通过采用多种统计方法,结合专家意见,筛选出敏感性、代表性、独立性及内部一致性均较好的条目。

Objective To develop a patient reported syndrome scale of Yin deficiency syndrome(PRS-YDS),select and optimize the items by multivariate statistical analysis.Methods Preliminarily develop a patient reported syndrome scale of Yin deficiency syndrome,scene investigation healthy peoples and patients with Yin deficiency syndrome.Items were selected based on comprehensively analyzed from semanteme,ceiling and floor effect,discrete trend method,correlation coefficient,cronbach's alpha coefficient,factor analysis and T test method.Results 80 subjects were asked to complete the scale,including 60 patients with Yin deficiency syndrome and 20 healthy subjects.The above statistical methods are used to analyze and summarize the results of the scale.The item"Sudden Face Fever"was deleted by two statistical methods at the same time.Combining with the expert opinion,it is considered that the item"sudden heat of the face"is similar to"sudden burst of heat",and the latter includes the former.Therefore,the item"sudden heat of the face"was deleted.PRS-YDS included three dimensions and ten items.Conclusion By a variety of statistical methods combined with expert opinions,the better items were selected which have good sensitivity,representativeness,independence and internal consistency.
