跨境缅甸的中国籍女性性工作者高危行为特征及影响因素分析

Analysis on the high-risk behaviors and the influencing factors of Chinese cross-border female sex workers in Myanmar

目的了解在缅甸的中国籍女性性工作者接待不同国籍嫖客高危行为,安全套使用情况及影响因素,为跨境艾滋病防控工作提供依据。方法 2017年7~9月,以缅甸掸邦第四特区为调查点,采用典型抽样的方式纳入中国籍跨境女性性工作者,知情同意后由经培训的当地医生进行现场问卷调查。收集人口学、性行为及安全套使用等信息,并采集血样进行HIV检测。通过单因素和多因素logistic回归分析接待不同国籍嫖客的影响因素及安全套的使用情况。结果共调查149人,平均年龄为(23.95±4.00)岁,69.1%为单身,49.7%为小学及以下文化程度。其中同时接待中缅籍嫖客118人,仅接待中国籍嫖客31人,平均每周接客数(26.82±11.28)人。选择接待不同国籍嫖客与跨境次数、性工作年限、每周接客频次以及是否接受过艾滋病、性病宣传有关。接待商业嫖客及与临时性伴、固定性伴性生活时,接待中缅籍嫖客的女性性工作者坚持每次都使用安全套的比率分别为81.7%,94.3%,86.8%,均大于接待纯中国籍嫖客者。本次调查未发现HIV阳性者。结论中国籍跨境女性性工作者中有大部分同时接待中缅籍嫖客,高危性行为明显,是区域艾滋病传播的关键人群。

Objective To understand the high-risk behaviors of Chinese cross-border female sex workers(FSWs) serving the clients with different nationalities in Myanmar,the status of condom use and the influencing factors,and provide a basis for cross-border HIV/AIDS prevention and control.Methods From July to September2017,the fourth Special District in Myanmar was selected as the survey point,and Chinese cross-border FSWs were recruited by typical sampling.A field questionnaire survey was carried out by the trained local doctors after the informed consent was obtained.The information of demographic characteristics,sexual behavior,and condom use were collected,and the blood sample was collected for HIV testing.Univariate and multivariate logistic regression analysis were used to analyze the influencing factors of FSWs receiving clients with different nationalities and the use of condom.Results A total of149 respondents were investigated.The mean age was23.95±4.00 years old,69.1% were unmarried,and49.7% had education level below elementary school.Among the participants,118 serviced Chinese and Burmese clients,and 31 only serviced Chinese clients.The average number of client was 26.82±11.28 per week.The cross-border frequency,length to be a sex work,weekly clients,and whether received HIV/AIDS education were related to Chinese FSWs serving the clients with different nationalities.The proportions of insisting on condom use among FSWs serving Chinese and Burmese clients in sex with commercial,temporary,and fix sex partners were 81.7%,94.3%,and 86.8%,which were all higher than those serving Chinese clients only respectively.None of the participents was HIV positive.Conclusion Many Chinese cross-border FSWs service Chinese and Burmese clients at the same time and have an obvious high-risk sexual behavior,which are the key population in regional HIV/AIDS transmission.
