改良温药灸包联合推拿点穴治疗小儿肠系膜淋巴结炎的临床观察

Clinical effect of moxibustion package with modified warming medicine combined with massage and acupoint-pressing in the treatment of mesenteric lymphadenitis in children

目的观察改良温药灸包联合推拿点穴治疗小儿肠系膜淋巴结炎(ML)的临床疗效。方法选取2019年1月-2019年7月广东省肇庆市端州区华佗医院门诊收治的ML患儿62例为研究对象,应用随机、对照研究方法,采用随机抽样软件将符合纳入标准的患儿随机分为治疗组、对照组,均31例,对照组予以西医常规治疗,治疗组在此基础上采用改良温药灸包联合推拿点穴治疗,比较两组临床疗效、主要症状消失时间、高频超声结果(淋巴结纵径、横径、纵横径比)、不良反应发生率。结果治疗组有效率(87. 10%)明显高于对照组(64. 52%)(P <0. 05);治疗组呕吐消失时间、腹泻/腹痛、食欲不振消失时间[(1. 28±0. 14) d、(3. 25±0. 36) d、(3. 56±0. 39) d]短于对照组[(1. 67±0. 19) d、(5. 13±0. 54) d、(4. 51±0. 47) d](P <0. 05);治疗组治疗后纵径(6. 43±0. 69) mm、横径(3. 26±0. 34) mm低于对照组(6. 93±0. 67) mm、(3. 51±0. 41) mm(P <0. 05),但两组纵横径比比较差异无统计学意义(P>0. 05);两组不良反应发生率(9. 68%vs 6. 45%)比较差异无统计学意义(P> 0. 05)。结论改良温药灸包联合推拿点穴治疗小儿ML疗效较好,可有效缓解其症状,改善淋巴结纵径、横径,且安全可靠,值得在临床推广实践。

Objective To observe the clinical effect of modified warm medicine moxibustion bag combined with massage and acupoint-pressing on mesenteric lymphadenitis( ML) in children. Methods 62 children with ML admitted to Huatuo Hospital,Duanzhou District,Zhaoqing City,Guangdong Province from January 2019 to July 2019 were selected as the research objects. The children who met the inclusion criteria were randomly divided into treatment group and control group by random sampling. The control group was treated with routine western medicine. The treatment group was treated with modified warm medicine moxibustion package on this basis. The clinical efficacy,the disappearance time of main symptoms,the results of high frequency ultrasound( lymph node longitudinal diameter,transverse diameter,aspect ratio) and the incidence of adverse reactions were compared between the two groups. Results The effective rate of the treatment group was significantly higher than that of the control group( 87. 10% vs. 64. 52%,P < 0. 05). The disappearance time of vomiting,diarrhea/abdominal pain and loss of appetite in the treatment group was shorter than that in the control group[( 1. 67 ± 0. 19 vs. 1. 28 ±0. 14) d,( 5. 13 ± 0. 54 vs. 3. 25 ± 0. 36) d,( 3. 56 ± 0. 39 vs. 4. 51 ± 0. 47) d;P < 0. 05]. The longitudinal and transverse diameters in the treatment group were lower than those in the control group[( 6. 43 ± 0. 69 vs. 6. 93 ± 0. 67) mm,( 3. 26 ±0. 34 vs. 3. 51 ± 0. 41) mm,( P < 0. 05),respectively],and there was no significant difference in the ratio of longitudinal and transverse diameters between the two groups( P > 0. 05). There was no significant difference in the incidence of adverse reactions between the two groups( 9. 68% vs. 6. 45%,P > 0. 05). Conclusion Modified warm medicine moxibustion package combined with massage and acupoint-pressing has a good effect on ML in children. It can effectively alleviate the symptoms,improve the longitudinal and transverse diameter of lymph nodes. It is safe and reliable and so worth popularizing in clinical practice.
