2型糖尿病患者高三酰甘油血症-腰围表型与糖尿病微血管并发症的相关性

The correlation between hypertriglyceridemic-waist phenotype and diabetic microvascular complications in patients with type 2 diabetes mellitus

目的探讨2型糖尿病患者高三酰甘油血症-腰围表型(HTWC)与糖尿病微血管并发症的相关性。方法选取内分泌科收治的2型糖尿病患者538例为研究对象。HTWC定义为腰围≥90 cm(男性)和85 cm(女性)且血三酰甘油≥1.7 mmol/L。结果539例调查人群中HTWC表型检出率为34.9%,糖尿病肾病和糖尿病视网膜病变的检出率为19.1%和15.6%。三酰甘油和腰围正常组糖尿病肾病检出率为10.6%,而HTWC组检出率为24.5%。校正性别、年龄、体重指数、高血压病和糖尿病病史后,HTWC组糖尿病肾病检出率是三酰甘油和腰围正常组2型糖尿病患者的2.516倍(95%CI:1.282~4.937,P=0.007)。但4组间糖尿病视网膜病变检出率比较差异无统计学意义(P>0.05)。结论HTWC表型与2型糖尿病患者早期肾病密切相关。

Objective To explore the correlation between hypertriglyceridemic-waist(HTWC)phenotype and diabetic microvascular complications in patients with type 2 diabetes mellitus(T2DM).Methods A total of 538 patients with T2DM who were treated in our hospital were enrolled in the study.The HTWC phenotype was defined as serum triglyceride concentrations≥1.7mmol/L and waist circumference≥90cm(males)and 85cm(females).Results Of the 538 patients with T2DM,the detection rate of HTWC phenotype was 34.9%,and the detection rate of diabetic nephropathy and diabetic retinopathy was 19.1%and 15.6%,respectively.The detection rate of diabetic nephropathy was 10.6%in T2DM patients with normal waist circumference and triglyceride,which was 24.5%in T2DM patients with HTWC phenotype.After adjustment for sex,age,body mass index,hypertension and history of diabetes,the detection rate of diabetic nephropathy in T2DM patients with HTWC phenotype was 2.516 times as high as that in T2DM patients with normal waist circumference and triglyceride(95%CI:1.282~4.937,P<0.01).Howeverthe there was no significant difference in the detection rate of diabetic retinopathy among the four groups(P>0.05).Conclusion There is a close correlation between HTWC phenotype and early nephropathy in patients with T2DM.
