浙江省2011-2018年艾滋病自愿咨询检测不同人群HIV抗体检测趋势分析

Trend analysis on HIV antibody screening testing for different populations in voluntary counseling and testing clinics from 2011 to 2018,Zhejiang province

目的了解2011—2018年浙江省艾滋病自愿咨询检测(voluntary counseling and testing,VCT)求询者HIV筛查阳性率和筛查阳性病例分类构成比趋势,为优化艾滋病防治策略和完成"90-90-90"防治目标提供依据。方法通过分析2011—2018年浙江省各VCT门诊数据,分析不同类型人群HIV抗体筛查阳性率和筛查阳性病例分类构成比情况,采用SAS 9.2软件,应用cochran-armitage trend方法进行趋势分析。结果2011—2018年共完成检测775324人次,发现筛查阳性8573人次,总体HIV筛查阳性率1.11%。HIV筛查阳性率呈上升趋势,从2011年的0.67%上升到2015年的1.30%,再到2018年的1.42%。配偶/固定性伴阳性、男男性行为的筛查阳性率呈下降趋势,商业异性性行为和非商业非固定异性性行为的筛查阳性率呈上升趋势。筛查阳性病例分类构成比中,注射吸毒史、配偶/固定性伴阳性、医疗相关问题和其他类型呈下降趋势,非商业非固定异性性行为和男男性行为呈上升趋势。结论自愿咨询检测服务作为检测发现工作的主要措施之一,需要及时分析目标人群性行为的变化特征,制定更有针对性的干预政策。

Objective To analyze the positive rate of screening for HIV and trend of constituent ratios in screening positive cases among patients in HIV/AIDS voluntary counseling and testing(VCT)clinics in Zhejiang province from 2011 to 2018,so as to provide evidences for HIV intervention strategy optimization and 90-90-90 solution.Methods The data in HIV VCT clinics were used to analyze the positive rate of antibody screening for HIV and the constituent ratios of different populations.The Cochran-Armitage trend method in SAS 9.2 software was used in trend analysis.Results From 2011 to 2018,775324 HIV screening tests were conducted.A total of 8573 HIV screening positive cases were found and the overall HIV screening positive rate was 1.11%.The rate showed a trend of increasing,from 0.67%in 2011 to 1.30%in 2015 and up to 1.42%in 2018.There were trends of decreasing in the rate of HIV positive spouse/regular partners and HIV screening positive rate among MSM while the screening positive rates among commercial heterosexual behaviors and casual partner heterosexual behaviors increased.For the constituent ratios of HIV screening positive cases,history of injected drug use(IDU),HIV positive spouse/stable sexual partner,clinic related issues and others showed a trend of decreasing while casual partner heterosexual behaviors and MSM showed a trend of increment.Conclusions As one of the most important measure of detection,VCT data should be timely analyzed for the characteristics of target population,so as to provide more specific interventio policies.
