浅谈临床药师加入家庭医生团队的作用

Role of Clinical Pharmacists Joining the Family Doctor Team

目的:社区临床药师加入家庭医生团队,通过对患者进行用药教育等方法,提高老年慢病患者(以合并糖尿病、高血压的患者为主要调查对象)的用药依从性,观察并探讨此工作模式是否可以提高药物疗效及降低不良反应发生率。方法:以深圳市罗湖集团下设社康中心65岁及以上老年慢病患者为观察对象,选取长期居住在本社区的159例老年患者,社区临床药师加入家庭医生团队,与家庭医生团队一起到老年患者家中进行走访、跟踪,并对其建立药学服务档案及进行用药教育,为患者提供药学相关知识。结果:家庭医生团队于2017年10月~2018年12月共跟踪服务了159例患者,社区临床药师服务后的老年慢病患者血压、血糖控制平稳,部分多病共存患者服药数量减少,患者的用药负担有所减轻。结论:临床药师参与家庭医生团队有助于患者获得更多正确的用药知识,提高患者的合理用药水平,减少潜在药害事件的发生。

Objective:To improve the drug compliance of elderly patients with chronic disease(mainly with diabetes and hypertension)by including the clinical pharmacists in the family medical team and providing drug education to patients.To observe and explore whether this working mode can improve the effect of drug therapy and reduce the incidence of adverse reactions.Methods:159 elderly patients aged 65 and above living in the Shenzhen Luohu Group community were selected as the observation objects.The community clinical pharmacists joined the family doctor team to visit elderly patients,and follow up the elderly patients with chronic diseases and provide pharmaceutical related education for patients.Results:From October 2017 to December 2018,the family doctor team followed up a total of 159 patients.The blood pressure and blood glucose control of the elderly patients with chronic illnesses were stable with the service of community clinical pharmacists,and the amount of drugs taken was reduced in some patients with multiple diseases.The financial burden of medication is reduced.Conclusion:The participation of clinical pharmacists in the family doctor team is helpful for patients to get more knowledge of rational drug use and reducing the occurrence of potential drug damage.
