妊娠期高血压疾病患者凝血功能指标及血小板参数的变化及意义

The Change and Significance of Platelet Parameters and Blood Coagulation Indexes in Pregnancy - induced Hypertension Syndrome Patients

目的探讨妊娠期高血压疾病患者凝血功能指标及血小板参数的变化及意义。方法选择50例妊娠期高血压疾病患者为疾病组,同期住院待产的正常晚期妊娠产妇50例为晚期妊娠组,以及50例健康孕龄妇女为对照组,比较各组凝血功能指标及血小板参数情况。结果疾病组、晚期妊娠组凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)均明显低于对照组,纤维蛋白原(FIB)明显高于对照组(P<0.05),但疾病组降低及升高程度明显大于晚期妊娠组(P<0.05)。各组凝血酶凝结时间(TT)比较差异无统计学差异(P>0.05)。疾病组、晚期妊娠组血小板计数(PLT)、血小板压积(PCT)均明显低于对照组,平均血小板体积(MPV)、平均血小板体积(PDW)均明显高于对照组(P<0.05),且疾病组MPV、PDW明显高于晚期妊娠组(P<0.05)。结论妊娠期高血压疾病患者存在明显的血小板活化及血液高凝状态,产前行凝血指标及血小板参数测定对于及时发现妊娠期高血压疾病具有重要指导意义。

Objective To explore the change and significance of platelet parameters and Blood coagulation indexes in Pregnancy-induced hypertension syndrome patients.Methods Selected 50 cases of PIHS patients as PIHS group,50 cases of normal late pregnancy as late pregnancy group,50 cases of healthy women as control group.To determinate and compare each coagulation index,platelet parameters.PIHS group of pregnant women may,in accordance with the illness severity was divided into mild,moderate,severe.To determinate and compare each coagulation index,platelet parameters.Results PIHS group of PT,APTT,late pregnancy group were significantly lower than the control group,FIB is significantly higher than control group(P<0.05).PIHS group higher than the control group,the reduction of the blood coagulation indexes of the more obvious(P<0.05).With the increase of PIHS severity,PT,APTT,TT is gradually decreasing trend(P<0.05),FIB showed a trend of increase gradually(P<0.05).PIHS group,the PLT,PCT late pregnancy group was obviously lower than the control group,MPV and PDW were significantly higher than that of control group(P<0.05).MPV and PDW PIHS group was obviously higher than that of the late pregnancy group(P<0.05).With the increase of PIHS severity,PLT,PCT value gradually reduced(P<0.05),MPV and PDW values increased(P<0.05).Conclusions PIHS state before pregnant women exist obvious thrombosis,prenatal line coagulation indexes and platelet parameters determination to discover the PIHS and illness severity has important guiding significance.
