基于目标策略的针对性护理干预对暴发性心肌炎患者的影响

The Effect of Strategy-based Targeted Nursing Intervention on Patients with Fulminant Myocarditis

目的:探讨基于目标策略的针对性护理干预对暴发性心肌炎患者的影响。方法:选取2013年1月1日~2019年2月28日收治的暴发性心肌炎患者43例为研究对象,根据护理方式不同分为对照组22例和观察组21例,对照组进行常规护理干预,观察组在常规护理基础上进行基于目标策略的针对性护理干预;比较两组临床疗效、心肌损伤程度[心肌肌钙蛋白T(cTnT)、肌酸激酶同工酶(CK-MB)、B型脑钠肽(BNP)]、心功能指标[左心室内径(LV)、左心室射血分数(LVEF)]、负性情绪[采用医院焦虑抑郁量表(HADS)]及生活质量[采用世界卫生组织生存质量测定量表(WHOQOL-100)]。结果:观察组临床疗效优于对照组(P<0.05);干预1个月后,两组cTnT、CK-MB、BNP、LV、LVEF、HADS-A、HADS-D及WHOQOL-100评分均优于干预前(P<0.05),且观察组优于对照组(P<0.05,P<0.01)。结论:基于目标策略的针对性护理干预能够提高暴发性心肌炎患者的临床疗效,改善患者心肌损伤程度、心功能、焦虑及抑郁等负性情绪,从而有效提高患者生活质量。

Objective:To explore the effect of strategy-based targeted nursing intervention on patients with fulminant myocarditis.Methods:A total of 43 patients with fulminant myocarditis admitted from January 1,2013 to February 28,2019 were selected as the research objects.According to the different nursing methods,they were divided into the control group(22 cases)and the observation group(21 cases).The control group received routine nursing intervention,and the observation group received targeted nursing intervention based on the conventional nursing.The clinical curative effect,the degree of myocardial injury(cTnT、CK-MB、BNP),cardiac function index(LV、LVEF),negative emotions(HADS)and the quality of life(WHOQOL-100)were compared between the two groups.Results:the clinical efficacy of the observation group was better than that of the control group(P<0.05).After 1 month of intervention,cTnT,CK-MB,BNP,LV,LVEF,HADS-A,HADS-D and WHOQOL-100 scores in both groups were better than before intervention(P<0.05),and the observation group was better than the control group(P<0.05,P<0.01).Conclusion:strategy-based targeted nursing intervention can improve the clinical efficacy of patients with fulminant myocarditis,and reduce the degree of myocardial injury,cardiac function,anxiety,depression and other negative emotions,so as to effectively improve the quality of life of patients.
