小剂量舒芬太尼在剖宫产术麻醉中预防寒战和牵拉痛的临床疗效探究

Small dose Sufentanil in Cesarean Section Anesthesia in the Prevention of Chills and Pull Pain Clinical Efficacy

目的分析小剂量舒芬太尼在剖宫产术麻醉中预防寒战与牵拉痛的临床疗效。方法选取2017年1月至2018年12月184例医院产科行择期剖宫产术的产妇,采取随机分组的方式将入选184例产妇分为观察组(92例)与对照组(92例)。对照组术中使用罗哌卡因镇痛,观察组在对照组的基础上加入小剂量舒芬太尼。观察两组产妇不同时间(手术前-T0、用药1 min后-T1、用药10 min后-T2、手术结束时-T3)的心率、平均动脉压、寒战以及牵拉痛情况。同时比较两组产妇在手术时间、麻醉苏醒时间以及新生儿Apgar评分等方面差异。结果两组产妇在T0、T1、T2、T3的心率和平均动脉压比较差异无统计学意义(P>0.05);观察组T1、T2、T3寒战评分与牵拉痛评分明显低于对照组(P<0.05);两组产妇在手术时间、麻醉苏醒时间以及新生儿Apgar评分比较差异无统计学意义(P>0.05)。结论剖宫产术中麻醉加入小剂量舒芬太尼的应用能够有效改善寒战和牵拉痛情况,且对产妇的生命体征、手术时间以及新生儿均无明显影响,具有较高的安全性。

Objective To analyze the clinical effect of low-dose sufentanil in preventing chills and traction pain during cesarean section anesthesia.Methods 184 women who underwent elective cesarean section in the obstetrics department of the hospital from January 2017 to December 2018 were selected.The 184 women were randomly divided into the observation group(92 cases)and the control group(92 cases).In the control group,ropivacaine was used for analgesia,and in the observation group,a small dose of sufentanil was added on the basis of the control group.Heart rate,mean arterial pressure,chills and traction pain were observed at different time(-T0 before surgery,-T1 after 1 min of medication,-T2 after 10 min of medication,and-T3 at the end of surgery).Meanwhile,the operation time,anaesthesia recovery time and newborn Apgar score were compared between the two groups.Results There was no significant difference in heart rate and mean arterial pressure between the two groups(P>0.05).The results of T1,T2 and T3 in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference between the two groups in operation time,anesthesia recovery time and neonatal Apgar score(P>0.05).Conclusion Adding small dose of sufentanil to anesthesia during cesarean section can effectively improve the situation of chills and traction pain,and has no obvious influence on the maternal vital signs,operation time and newborn,so it has a high safety.
