呼气末CO2监测辅助ICU患者置鼻胃管中的应用研究 被引量:3

摘要 目的观察呼气末CO2监测辅助ICU患者置鼻胃管中的应用效果。方法选取2018年11月-2019年9月间,需置鼻胃管的120例患者,随机分为试验组和对照组。试验组采用呼气末二氧化碳传感器辅助置鼻胃管方式。对照组采用传统盲插鼻胃管方式。比较两组一次性置管的成功率、置管的时间、置管操作中并发症的发生率以及患者置管中的生命体征的变化。结果试验组一次性置管成功率高于对照组(P<0.05)。试验组护士置管时间比对照组护士置管时间短(P<0.05)。试验组ICU患者在鼻胃管置管操作中并发症发生率比对照组低(P<0.05)。当鼻胃管进入食道后,试验组患者的心率、舒张压、收缩压均比对照组低,血氧饱和度比对照组高(P<0.05),试验组患者的心率、舒张压、收缩压均高于操作前(P<0.05)。结论呼气末CO2监测辅助ICU患者置鼻胃管中的能够提高护士一次性置管成功率,缩短置管时间,降低置管操作过程中的并发症发生率,维持患者生命体征平稳,此方法简单、准确、安全、可检测。 Objective To observe the effect of end-tidal CO2 monitoring in the treatment of nasogastric tube in ICU patients.Methods 120 patients with nasogastric tube were randomly divided into the test group and the control group in the period from November 2018 to September 2019.The test group uses the end-tidal carbon dioxide sensor to assist the nasogastric tube mode.The control group adopts the traditional blind-insertion nasogastric tube method.The success rate of the one-time tube insertion,the time of the tube insertion,the incidence of the complications in the operation of the catheter and the change of the vital signs in the patient’s catheter are compared.Results The success rate of the one-time insertion of the test group is higher than that of the control group(P<0.05).The catheterization time of the nurses in the test group was shorter than that in the control group(P<0.05).The incidence of complications in nasogastric catheterization in the experimental group was lower than that in the control group(P<0.05).When the nasogastric tube entered the esophagus,the heart rate,diastolic blood pressure and systolic pressure of the experimental group were lower than those of the control group,and the blood oxygen saturation of the experimental group was higher than that of the control group(P<0.05).The heart rate,diastolic blood pressure and systolic pressure of the experimental group were higher than those before operation(P<0.05).There was no significant difference in blood oxygen saturation between the two groups(P>0.05).Conclusion CO2 at the end of exhalation Monitoring and assisting patients with ICU can improve the success rate of one-off catheterization,shorten the time of catheterization,reduce the incidence of complications in the operation of catheterization,and maintain the stable vital signs of patients.This method is simple,accurate,safe,can be detected,and is worthy of clinical promotion.
