瘢痕子宫再次妊娠致子宫破裂危险因素分析

Analysis on the risk factors of uterine rupture caused by second pregnancy of cicatricial uterus

目的分析瘢痕子宫再次妊娠致子宫破裂的危险因素。方法回顾性分析我院2016年1月~2019年1月收治的42例瘢痕子宫再次妊娠发生子宫破裂的患者,再选取同期未发生子宫破裂的58例产妇作为研究对象,本研究对致子宫破裂的影响因素展开分析,并与未发生子宫破裂的产妇进行相关情况的比较,分析差异性。结果通过研究可知,产妇高龄、产次多、妊娠足月、妊娠间隔时间<2年、前次单层缝合方式、瘢痕厚度<0.3cm是瘢痕子宫破裂的独立危险因素(P<0.05);经比较,未发生子宫破裂组的自然分娩、剖宫产、新生儿窒息、新生儿Apgar评分均优于子宫破裂组(P<0.05)。结论应对瘢痕子宫再次妊娠的产妇多加宣传,增强其自我保护意识,减少子宫破裂的发生。

Objective To analyze the risk factors of uterine rupture caused by second pregnancy of cicatricial uterus.Methods Retrospective analysis was conducted on 42 patients with uterine rupture caused by second pregnancy of cicatricial uterus admitted and treated in our hospital from January 2016 to January 2019,and 59 parturients without uterine rupture during the same period were selected as the study subjects.In this paper,the influencing factors leading to uterine rupture were analyzed,and the relevant situations were compared with those of the parturients without uterine rupture to analyze the differences.Results According to the study,advanced maternal age,multiple parities,full term,gestation interval<2 years,previous single-layer suture method,and scar thickness<0.3cm were the independent risk factors for cicatricial uterine rupture(P<0.05);after comparison,natural delivery,cesarean section,neonatal asphyxia,and neonatal Apgar score in the group without uterine rupture were all better than those in the group with uterine rupture(P<0.05).Conclusion Parturients with second pregnancy of cicatricial uterus shall pay more attention to enhance their self-protection awareness and reduce the occurrence of uterine rupture.
