个案管理模式对提高子宫内膜异位症手术患者自我管理能力和受孕成功率影响的研究

Study on the influence of case management model on improving the self-management ability and the success rate of conception in patients with endometriosis surgery

目的探讨个案管理模式对提高子宫内膜异位症手术患者自我管理能力和受孕成功率影响。方法选取2016年9月~2018年9月我院收治的子宫内膜异位症手术患者120例,按照随机数字表法分为实验组和常规组,每组60例,常规组给予常规护理,实验组在基础上给予个案管理模式护理。比较两组月经恢复时间、护理前后的焦虑自评量表(SAS)、抑郁自评量表(SDS)等心理状况评价指标、护理前、护理6、12个月的受孕成功率和自我管理能力评分以及并发症发生率。结果实验组护理后月经恢复时间、SAS、SDS评分明显低于常规组,差异有统计学意义(P<0.05);随访1年后,实验组护理6、12个月的受孕成功率和自我管理能力评分明显高于常规组,差异有统计学意义(P<0.05);实验组护理后的并发症明显低于常规组,差异有统计学意义(P<0.05)。结论个案管理模式能够有效改善子宫内膜异位症手术患者的月经情况和心理状态,有利于提高患者的自我管理能力和受孕成功率,且可降低术后并发症的发生,值得在临床上推广。

Objective To investigate the influence of case management model on improving the self-management ability and the success rate of conception in patients with endometriosis surgery.Methods 120 patients with endometriosis surgery admitted and treated in our hospital from September 2016 to September 2018 were selected,and were divided into the experimental group and the conventional group by using the random number methods,with 60 patients in each group.The conventional group was given routine nursing,while the experimental group was further given case management model nursing on the basis of conventional group.The menstrual recovery time,psychological status evaluation indexes before and after nursing such as self-rating anxiety scale(SAS),self-rating depression scale(SDS),etc.,success rate of conception,self-management ability score and incidence of complications before nursing and at 6 and 12 months of nursing were compared between the two groups.Results The menstrual recovery time,SAS and SDS scores after nursing in the experimental group were significantly lower than those in the conventional group,and the difference was statistically significant(P<0.05);After 1 year of follow-up visit,the success rate of conception and self-management ability score at 6 and 12 months of nursing in the experimental group were significantly higher than those in the conventional group,and the difference was statistically significant(P<0.05);The incidence of complications after nursing in the experimental group was significantly lower than that in the conventional group,and the difference was statistically significant(P<0.05).Conclusion The case management model can effectively improve the menstrual status and psychological status of patients with endometriosis surgery,improve the patients’self-management ability and success rate of conception,and reduce the incidence of postoperative complications,which is worthy of further promotion in clinical practice.
