摘要
目的探讨血小板平均体积(MPV)、糖化血红蛋白(HbA1C)及超敏C反应蛋白(hs-CRP)水平监测在2型糖尿病微血管病变中的临床应用。方法回顾性分析我院2018年2月~2019年6月受检者性别、年龄、有无糖尿病及微血管病变病史等基本资料,将收治的120例2型糖尿病患者纳入2型糖尿病组,进一步按血糖控制情况及有无微血管病变分为A组、B组和C组,另外选取40例健康体检者作为健康对照组。所有研究对象分别采用日本SYSMEX全自动血细胞分析仪、美国Bio-rad公司糖化血红蛋白仪、BECKMAN COULTER公司IMMAGE 800特定蛋白分析仪、美国Abbott公司i2000SR免疫分析仪、深圳Snibe公司MAGLUMI 2000Plus化学发光仪测定MPV、HbA1C、hs-CRP、Hcy、U-mAlb水平并进行比较。结果2型糖尿病组MPV、HbA1C、hs-CRP、Hcy、U-mAlb水平高于健康对照组(P<0.05);2型糖尿病组MPV、HbA1C、hs-CRP、Hcy、U-mAlb水平呈C组>B组>A组(P<0.05)。各指标联合检测诊断早期糖尿病肾病的敏感度、特异度均较单独检测提高。结论MPV、HbA1C及hs-CRP水平的监测对于2型糖尿病微血管病变的早期诊断具有重要价值。
Objective To explore the clinical application of mean platelet volume(MPV),glycated hemoglobin(HbA1C),and high-sensitivity C-reactive protein(hs-CRP)levels monitoring in type 2 diabetic mellitus(T2DM)microangiopathy.Methods The basic data such as gender,age,history of diabetes and microangiopathy of patients treated in our hospital from February 2018 to June 2019 were analyzed retrospectively.120 patients with T2DM were included in T2DM group.They were divided into group A,group B and group C according to the blood glucose control status and the presence or absence of microangiopathy.Meanwhile 40 healthy subjects were selected as healthy control group.The MPV,HbA1C,hs-CRP,Hcy,U-mAlb levels of all study subjects were measured by Japanese SYSMEX automatic blood cell analyzer,American Bio-rad company glycated hemoglobin analyzer,BECKMAN COULTER company IMMAGE 800 specific protein analyzer,American Abbott company i2000SR immunoanalyzer,Shenzhen Snibe company MAGLUMI 2000Plus chemiluminescence meter and compared.Results The MPV,HbA1C,hs-CRP,Hcy,and U-mAlb levels in the T2DM group were higher than those in the healthy control group(P<0.05);The MPV,HbA1C,hs-CRP,Hcy,and U-mAlb levels in the T2MD group showed group C>group B>group A(P<0.05).The sensitivity and specificity of the combined detection of all indicators in the diagnosis of early diabetic nephropathy were higher than those of the individual detection.Conclusion The monitoring of MPV,HbA1C and hs-CRP levels is of great value for the early diagnosis of T2DM microangiopathy.
