前庭自旋转试验在中枢血管性急性前庭综合征中的应用 被引量:1

摘要 目的:探讨前庭自旋转试验(VAT)对中枢血管性急性前庭综合征的诊断价值。方法:选择齐齐哈尔医学院附属第三医院2016年11月至2018年12月来诊的中枢血管性急性前庭综合征患者116例,男47例,女69例,年龄32~79岁;病程1~10 d。全部患者均行MRI或CT检查,其中后循环短暂性脑缺血发作(TIA)97例,小脑梗死19例。所有患者均采用VAT技术进行检测,分析其检测结果。结果:VAT检查总异常率为86.2%(100/116例),其中后循环TIA组异常率为85.6%(83/97例),小脑梗死组异常率为89.5%(17/19例)。VAT主要表现水平增益增高或不同频段水平增益增高与减低并存,水平或垂直相移增高或减低。结论:VAT对诊断中枢血管性急性前庭综合征有重要价值,VAT结合MRI或CT可使前庭系统损害的诊断定位与前庭功能评估更加完善。 Objective:To evaluate the value of vestibular autorotation test(VAT)in the diagnosis of central acute vestibular syndrome(CAVS).Methods:One hundred and sixteen patients with CAVS in the Third Affiliated Hospital of Qiqihar Medical College from November 2016 to December 2018,including 47 males and 69 females,aged 32-79 years,with a course of 1-10 days,were enrolled in the study.MRI or CT was performed in all patients diagnosed as TIA with 97 cases and cerebellar infarction with 19 cases.All patients were tested with VAT technology and the results were analyzed.Results:The total abnormal rate of VAT was 86.2%(100/116 cases).The abnormal rate was 85.6%(83/97 cases)in TIA group and 89.5%(17/19 cases)in cerebellar infarction group.VAT showed the increase of horizontal gain or the coexistence of increase and decrease of horizontal gain in different frequency bands,and the increase or decrease of horizontal or vertical phase shift.Conclusion:VAT is of great value in the diagnosis of central vascular acute vestibular syndrome.VAT combined with MRI or CT can improve the diagnosis and location of vestibular system lesions and vestibular function assessment.
