DWI对前列腺中央腺体局灶性病变良恶性的鉴别诊断价值 被引量:1

摘要 目的探讨1.5 TMR弥散加权成像(DWI)对前列腺中央腺体局灶性病变良恶性的鉴别诊断价值。方法选择2017年1月〜2019年12月在广东省信宜市人民医院接受诊治的前列腺中央腺体局灶性病变患者90例,均经手术穿刺活检获得明确的病理诊断,根据病理诊断结果分组,前列腺中央腺体局灶性良性病变46例,前列腺中央腺体局灶性恶性病变44例,全部研究对象均予以术前1.5 T磁共振表观弥散系数(ADC)及DWI检查,比较前列腺中央腺体局灶性良性病变及恶性病变的DWI图像信噪比(SNR)、病灶的ADC值(b=0.1000 s/mm^2)。采用t检验评估两组的DWI半定量分级指标,统计学方法采用卡方检验。结果前列腺中央腺体局灶性良性病变的SNR显著低于恶性病变组(P<0.05);前列腺中央腺体局灶性良性病变的病灶ADC值显著高于恶性病变组(P<0.05);前列腺中央腺体局灶性病变良性组的DWI半定量分级1〜3级29例,4〜5级17例;恶性组1〜3级13例,4〜5级31例,两组比较差异有统计学意义(P<0.05)0结论1.5 T磁共振DWI用于鉴别诊断前列腺中央腺体局灶性病变良恶性的诊断价值更高。 Objective To evaluate the value of DWI with different diffusion sensitivity coefficient on a 1.5 T MR scanner in the differential diagnosis of benign and malignant lesions in prostate central gland.Methods From January 2017 to December 2019,90 patients with localized prostate gland lesions were recruited in our hospital.All of them got confirmed pathological diagnosis.Based on that,the patients were divided into two groups,one with 46 cases of localized prostate gland benign lesions,and one with 44 cases of localized prostate gland malignant lesions.All of the subjects were scanned preoperative on a 1.5 T MR scanner and apparent diffusion coefficient(ADC)and diffusion weighted imaging(DWI)data were obtained.Signal-to-noise ratio(SNR)and ADC value(b=0,1000 s/mm^2)of DWI images in benign and malignant lesions of prostate central gland were compared with仁test.Chi square test was used to evaluate the DWI semi-quantitative grading index.Results The signal-to-noise ratio(SNR)of DWI images of prostate central gland localized benign lesions was significantly lower than that of malignant lesions(P<0.05).The ADC value of the focus of prostate central gland localized benign lesions was significantly higher than that of malignant lesions(P<0.05).The DWI semi quantitative grading of the benign group was 1〜3 in 29 cases,4-5 in 17 cases,1〜3 in 13 cases,and 4〜5 in 31 cases,the difference between the two groups was significant(P<0.05).Conclusions DWI with different diffusion sensitivity coefficient on 1.5 T MR scanners is more valuable in the differential diagnosis of benign and malignant lesions of central prostate gland.
