血清LAMB1在结直肠癌患者中的表达及其临床意义

Expression of serum LAMB1 and its clinical significance in patients with colorectal cancer

目的探究血清层粘连蛋白-1(LAMB1)在结直肠癌患者中的表达及其临床意义。方法收集在我院普通外科住院并符合纳入及排除标准的结直肠癌患者及无结直肠癌的同期住院患者,分为结直肠癌组和对照组,每组80例,通过酶联免疫法(ELISA)检测受试者血清中的LAMB1和癌胚抗原(CEA),并分析其对结直肠癌诊断的效果,观察LAMB1的浓度与病理分型的关系。结果结直肠癌组患者血清中的LAMB1和CEA显著高于对照组(P<0.05)。结直肠癌患者血清中的LAMB1和CEA与性别、年龄、肿瘤直径及肿瘤类型无相关性(P>0.05),但与肿瘤浸润深度及TNM分期均呈正相关,其与结直肠癌分化程度呈负相关,单分析LAMB1、单分析CEA及同时分析LAMB1与CEA对结直肠癌诊断时的ROC曲线下面积(AUC)分别为0.887、0.842及0.901,LAMB1鉴别结直肠癌患者与无结直肠癌患者(对照组)优于CEA。此外,LAMB1联合CEA可进一步提高诊断性能。结论 LAMB1在结直肠癌患者血清中的表达情况可反映患者病理分型,且有望作为结直肠癌诊断的一种标志物。

Objective To investigate the expression of serum LAMB1 and its clinical significance in patients with colorectal cancer. Methods 80 patients with colorectal cancer hospitalized in the Department of General Surgery and met inclusion and exclusion criteria of colorectal cancer(colorectal cancer group) and 80 inpatients without colorectal cancer(control group) in the same time were collected. ELISA method was used to detect the levels of serum LAMB1 and CEA, the effect for diagnosis of colorectal cancer was analyzed. The relationship between the level of LAMB1 and the pathological classification was observed. Results The levels of serum LAMB1 and CEA in the colorectal cancer group were significantly higher than those in the control group(P<0.05). The levels of serum LAMB1 and CEA in the colorectal cancer group were not related with gender, age, tumor diameter and tumor types(P>0.05), but they were positively related with tumor infiltration depth and TNM stage, and they were negatively related with the differentiation degree of colorectal cancer. The AUC of single analysis LAMB1, single analysis CEA and simultaneous analysis of LAMB1 and CEA in the diagnosis of colorectal cancer were 0.887, 0.842 and 0.901, respectively. LAMB1 was superior to CEA in distinguishing colorectal cancer patients and control group patients. In addition, LAMB1 combined with CEA could further improve the diagnostic performance.Conclusion The level of serum LAMB1 in colorectal cancer can distinguish the pathological classification of colorectal cancer patients, and it is expected to be a marker for the diagnosis of colorectal cancer.
