摘要
目的:对比酪酸梭菌二联活菌散与布拉酵母治疗新生儿肺炎合并抗生素相关性腹泻的效果。方法:选取2016年1月至2018年7月收治的94例肺炎合并抗生素相关性腹泻新生儿,随机分为观察组和对照组,观察组患儿使用酪酸梭菌二联活菌散治疗,对照组患儿使用布拉酵母治疗,比较两组患者的临床疗效、腹泻情况、住院时间、治疗前后肠道菌群指标、治疗前后促炎因子水平。结果:治疗后观察组患儿临床疗效总有效率为91.49%,高于对照组的74.47%(P<0.05);两组患儿使用抗生素至出现腹泻时间差异无统计学意义(P>0.05);观察组患儿止泻时间、住院时间较对照组缩短(P<0.05);治疗前两组患儿肠道菌群指标差异无统计学意义(P>0.05);治疗后两组患儿肠道乳杆菌、双歧杆菌、真杆菌数量均上升,且观察组上升更高(P<0.05);治疗前两组患儿促炎因子水平差异无统计学意义(P>0.05);治疗后两组患儿促炎因子水平均下降,且观察组患儿的IL-2、IL-6、TNF-α水平下降更多(P<0.05)。结论:酪酸梭菌二联活菌散治疗抗生素相关性腹泻肺炎新生儿效果较好,缩短止泻时间,调节肠道菌群,抑制促炎因子水平,促进肠道功能恢复,值得在临床上推广。
Objective:To compare the effects of combined clostridium butyricum and bifidobacterium powders,live and saccharomyces boulardii in the treatment of neonatal pneumonia with antibiotic-associated diarrhea.Methods:94 neonates with pneumonia and antibiotic-associated diarrhea admitted from January 2016 to July 2018 were chosen and randomly divided into the observation group and the control group.The observation group were treated with combined clostridium butyricum and bifidobacterium powders,live,while the control group were treated with saccharomyces boulardii,with the clinical efficacy,diarrhea,hospitalization time before and after treatment,intestinal floras,proinflammatory factor levels before and after treatment compared.Results:The total effective rate was 91.49%in the observation group,significantly higher than that in the control group(74.47%)(P<0.05).There was no significant difference between the two groups in the time of starting using antibiotics to diarrhea(P>0.05).The hospitalization time and the antidiarrheal time of the observation group were significantly shorter than those of the control group(P<0.05).There was no significant difference in intestinal floras between the two groups before treatment(P>0.05).After treatment,the number of lactobacilli,bifidobacteria and eubacteria all increased in both groups,with that of the observation group significantly larger than that of the observation group(P<0.05).There was no significant difference in the levels of pro-inflammatory factors between the two groups before treatment(P>0.05).The levels of pro-inflammatory factors in both groups decreased significantly,with the levels of IL-2,IL-6 and TNF-αin the observation group decreased more significantly(P<0.05).Conclusion:Combined clostridium butyricum and bifidobacterium powders,live is very effective in the treatment of neonatal pneumonia with antibiotic-associated diarrhea,which can shorten the time of diarrhea,regulate intestinal floras,inhibit the levels of pro-inflammatory factors and promote the recovery of intestinal function,worthy of clinical promotion.
